POWER AUTHORITY OF THE STATE OF NEW YORK JAMES A. FITZPATRICK NUCLEAR POWER PLANT DOCKET NO. 50-333

.srTALHMENT TO LER 79-095/03X-1 Page 1 of I During normal operation on October 25, 1979, while performing F-ISP-4-5, ti tled "Drywell Suppression Chamber Dif ferential Pressure,"

drywell to suppression chamber differential pressure transmitter 27-PT-102B was found out of tolerance in the conservative direction.

The instrument was being calibrated as required by Technical Specifications, Appendix A, Table 3.2.6 and was in error approximately 2% as compared to a procedural limit of 0.5%. Redundant di fferential pressure instrument channels were veri fied fully operable, 27-PT-102B was calibrated and returned to service. In addition, the surveillance interval was increased from once each 6 months to orce each week.

On November 1,1979, the instrument was found in error approximately 2% in the non-conservative direction and was re-calibrated and the surveillance interval was increased to once each day.

During the check on November 2, 1979, the instrument was again found out of calibration in the non-conservative direction. At this time the ins trument was declared inoperable. The instrument was replaced on November 16, 1979 and evaluation of the new instrument and the calibration procedures was undertaken. This evaluation revealed that during a portion of the test a " Gauge" pressure instrument rather than an " Absolute" pressure instrument was specified by the procedure. The procedure (F-ISP-4-5) was corrected and proper calibration of 27-PT-102B was verfied.

On November 29,1979, 27-PT-102B was veri fied fully operable and returned to service.

NOTE: Revision l'of this LER is submitted to correct items 12, 13, 14, 15, 17, 18, 19, 24, 25, 26, and this attachment.
