APPENDIX A NOTICE OF VIOLATION License No. DPR-72 Florida Power Corporation Crystal River Nuclear Plant 3 Based on the NRC inspection June 14-15, 1979, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. - These items have been categorized as described in correspondence to you dated December 31, 1974.
A.
As required by 10 CFR 50.54(i-1), the licensee shall not make a change to the accepted requalification training program by which the scope, frequency, or time alloted to conducting parts of the program is reduced without prior approval of the Commission.
Contrary to the above, the licensee reduced the requirements for on-shift participation for backup licensed operators from five days every four months to two days every three months.
In addition, eight (8) backup licensed operators failed to meet either the 15 days per year or the 8 day per year requirement during the period from January 1, 1978, through April 1, 1979.
This is an Infraction.
As required by 10 CFR 55, Appendix A, lectures shall be conducted where B.
performance on annual examinations indicates that additional emphasis is needed. The accepted requalification program stipulates that only those licensees who score greater than 80% on each section of the annual examina-tion may be exempted from participation in the lecture (s) given on that section.
Contrary to the above, one licensed reactor operator and one licensed senior reactor operator scored less than 80% on a section of the 1978 annual examination administered in January of 1978 and they failed to attend a lecture covering that section during the period before the 1979 annual examination.
This is a Deficiency.
C.
As required by Technical Specification 6.8.1, procedures shall be written and implemented.
Contrary to the above, Procedure Al-800, Item 6.1, was not implemented in that required semi-annual evaluations were not prepared for eight (8) licensed backup operators for one or both semi-annual periods in 1978.
This is a Deficiency.

As required by 10 CFR 19.12, all individuals working in or frequenting any portion of a restricted area shall be instructed to report promptly to the licensee any condition which may cause or lead to a violation of Commission regulations.
Contrary to the above, the lesson plan for instructions to workers did not include such instruction material and one of six workers interviewed on June 14, 1979, stated that he was unaware of this requirement.
This is a Deficiency.
