Conditions Prior to Event

The unit was in Mode 5.

Description of Event

During a surveillance test schedule review on 9/6/79, it was determined that FNP-1-STP-605.4 (Auxiliary Building Battery Quarterly Verification) had been performed on 6/27/79 (2 days beyond the GTP frequency of 92 days

+ 25%).

Tech. Spec. 4.8.2.4.a requires the surveillance test to be performed within a time interval of 92 days.

Designation of Apparent Cause:

This occurrence is attributable to personnel error.

Analysis of Event

An investigation of this incident determined the following:

a.

Review of the Surveillance Test Schedule on 6/2/79 indicated that the surveillance interval was due and personnel responsible for the performance of the STP were notified by Operations personnel.

The surveillance test was not performed.

The missed surveillance was discovered on 6/27/79, an LCO was initiated, the surveillance test was successfully performed and the surveillance test schedule initialed.

b.

An LCO Status Sheet was completed, however, it apparently was lost when it was transmitted to Operations Supervision.

Due to loss of the LCO Status Sheet, personnel responsible for LER preparation were unaware of the incident until 9/6/79.

c.

The health and safety of the public were not affected by this event.

Corrective Action

The necessity of performing surveillance tedts on their scheduled due dates will be emphasized to t!aintenance Supervision.

Appropriate Operations personnel have been reinstructed as to the importance of reviewing Surveillance Schedules for lapsed surveillances. A revised method of notifying personnel responsible for LER preparation was initiated subsequent to this event but prior to 9/6/79 and is considered adequate for preventing late reporting.

Failure Data:
