No activity limits were violated.

ABNORMAL OCCURRENCE REPORT Palisades Plant

1. Report No: A0-75-17.
2. a. Report Date: August 12, 1975.
b. Occurrence Date: July 29, 1975 (Classified as an AO).
3. Facility: Palisades Nuclear Plant, Covert, MI.
4. Identification of Occurrence: Gaseous waste release with a shorter than permissible holdup period.
5. Conditions: Steady state power Bo percent.
6. Description of Occurrence: Waste gas decay tank T lOlC lo~t 4 pounds of pressure on 7/18/75 and 5 pounds of pressure on 7/28/75. Both pressure drops occurred over approximately 8-hour periods. In addition, gradual pressure drop of 2 pounds occurred over the 10-day period between 7/18/75 and 7/28/75. There were no waste gas system operations being conducted which could be associated with the loss in pressure .

Designation of Apparent Cause: Although a thorough investigation proved inconclusive, a small valve leak is considered to be the most probable cause.

Analysis of Occurrence: The total gaseous loss represented less than 1%

of the allowable yearly average limit. The pressure drop was reported in a timely manner and in accordance with plant procedures.

9. Corrective Action: The following tests or checks were performed in an effort to determine the cause of the pressure drop.
a. The pressure transmitter calibration was verified.
b. The tank was checked for the presence of significant quantities of water and none was found.
c. All valve bonnet bolting was checked.
d. Solenoid Valve SV-1162 and associated drain Valve CV-1162 B were checked with no apparent problem being observed.

Since we were unable to determine the cause of the pressure drop, the Radio-active Materials* Control Department will provide special moni taring of this tank during its next period of use .
