  • PALISADES PLANT - ER-76 CONTAINMENT AIR LOCKS Attached is an event report which relates to the Palisades Plant. This event concerns leakage problems which were encountered during testing or inspection of the personnel and escape air locks.

  • LICENSEE EVENT REPORT ER-76-36 Palisades Escape Lock During an investigation of the operation of the escape lock opening and closing mechanisms, the air lock inner door was observed to be incorrectly closing and that air was passing by the door when it was "closed." Investigation revealed that with the normal containment pressure (approximately 2 psig), and wi:Gh the lock equalized to outside pressure, the door seating (latching) mechanism was not adjusted properly to prevent air from escaping by the inner door seal.

The door sealing mechanism was adjusted and an effective seal obtained.

The outer door was functioning properly at the time of this event and it is also considered likely that should an accident have occurred, any significant increase in containment pressure would have caused the inner door to seal.

Personnel Lock During the local leak testing of the personnel lock, leakage was identified when

  • the loGk was pressurized to about 20 psig. The lock was depressurized and the leak repaired. The leakage was through a shaft seal associated with the outer door locking mechanism. Retesting of the lock was satisfactory.

A review of .this event disclosed that procedures were not followed properly in that an "as found leakage" was not determined. Action to correct this has been taken .
