  • SUPPLEMENT - EVENT REPORT ER-77-25 The boric acid heat tracing system consists of two separate, redundant circuits or "channels." Each circuit feeds identical piping runs, only some of which are necessary for safeguards flow paths. Temperature recorders provide moni-toring at various points along the heat traced piping.

Only one of the two heat tracing circuits is normally energized with the other serving as a backup and the two rotated periodically. In this event, elec-tricians received clearance tagging for repair of a portion of the backup channel. Neither electricians nor operations personnel recognized at that point the time requirement of Technical Specification 3.2.3.d. The condition remained until recognized by a Shift Supervisor prior to start-up from a cold shutdown on 5-19-77. Both circuits were then made operable prior to plant heatup. J' The normal administrative control of equipment tag out is an Equipment* Outage Request (EOR) form on which the Shift Supervisor denotes operability require-ments. The Maintenance Supervisor initiates this form based on the plant Q-List, but in this case, the heat tracing breaker was not Q-Listed and both parties missed the 24-hour requirement. Contributing factors were probably the history of regular required maintenance on individual points and the fact that one channel is normally turned off anyway. To tighten this con-trol, we have generated a condensed list of all Chapter Three equipment for screening use by Maintenance Supervisors and as a reference for Shift Super-visors.

The investigation of the specific problem with the heat tracing indicates some confusion with Technical Specification 3.2.3.d. We interpret this literally to refer to the entire channel out of service and will address individual point problems in decisions on minimum flow path operability of the other paragraphs in that section. Technical Specifications changes will possibly be requested to more clearly show the intent, such as the wording in the standard specifications.

Increased emphasis has been placed on maintenance of the heat tracing system and we are considering a modification to decrease the problems .
