10CFR50.59 EVALUATIONS DOCKET NO: 50-311 MONTH: JUNE 1996 UNIT NAME: SALEM2 CONTACT: R. HELLER TELEPHONE: 609-339-5162 The following items were evaluated in accordance with the provisions of the Code of Federal Regulations IOCFR50.59. The Station Operations Review Committee has reviewed and concurs with these evaluations.

1. DCP Packages (DCP) 2EE-0142 Pkg. 1 "Valve 2HY15 Replacement" Rev. 0 - The DCP involves replacement of a non safety related valve (2HYI 5) with a more reliable component. This component is not directly a part of a Technical Specification system and does not support the function of any other safety related systems.

This DCP does not affect the emergency boration paths or flow rates assumed in the design bases section of the Technical Specifications. The required rapid boration flow rates from the boric acid tanks are not affected by fluctuations in the Volume Control Tank cover gas pressure. Procurement of the new pressure regulator in "fail closed on loss of air" configuration, does not change the existing margin of safety of the Chemical and Volume Control system. There is no reduction in the margin of safety as defined in the bases for any Technical Specification. (SORC 96-025) 2EE-0143 Pkg. 1 "Permanent Installation of Reconstitution Basket" Rev. 0 -

This DCP includes permanent replacement of the new fuel elevator carriage (basket) with a specially designed reconstitution basket. The reconstitution basket is designed to handle new fuel as well as fuel repair activities.

Modification of the new fuel elevator does not affect any existing analysis which forms the bases for the Technical Specifications. Technical Specification 3.9.11 requires that at least 23 feet of water shall be maintained over the top of irradiated fuel assemblies seated it the storage racks. This requirement of maintaining 23 feet of water over the stored fuel assembly does not apply to an irradiated fuel assembly being repaired since it is not considered "stored". A depth of 10 feet of water, required for safe shielding during "fuel

(SORC 96-025) 2EE-0248, Pkg. 1 "Charging Pump Seal Housing Replacement" Rev. 0 - This DCP replaces the existing water-cooled outboard mechanical seal housing for No. 22 Charging/Safety Injection pump, with an air-cooled housing that does not require external cooling connections. The existing Component Cooling water connections to the outboard mechanical seal housing will be deleted. The operability or design basis performance of No. 22 Charging/Safety Injection pump is not affected by these modifications. The modifications do not impact any Emergency Core Cooling System or emergency boration flow paths. There is no reduction in the margin of safety as defined in the bases for any Technical Specification. (SORC 96-056) 2EC-3266, Pkg. 1 "DIG LIO Temperature Low Alarm Setpoint" Rev. 0 - This DCP accomplishes three tasks. First, 2A Emergency Diesel Generator (EDG) lube oil pump heater failure alarm temperature switch and lube oil heater control switch (0-4000F) are replaced with similar, 0-250°F range switches.

Temperature sensor locations on 2C EDG will be rearranged to facilitate consistent physical arrangement of both the heaters and heater failure alarm. Following the switch replacement and sensor rearrangement, the low temperature alarm setpoint will be raised from 80°F to 100°F for all three EDG's Lube Oil Heater Failure Alarm Low temperature setpoints. Neither the high temperature setpoints nor the heater on and off setpoints will be affected.

There is no reduction in the margin of safety as defined in the bases for any Technical Specification. (SORC 96-034)

EVALUATIONS DOCKET NO: 50-311 MONTH: JUNE 1996 UNIT NAME: SALEM2 CONTACT: R.HELLER TELEPHONE: 609-339-5162 (Cont'd) 2EC-3349 Pkgs. 1 & 2 "2A & 2B EDGs Starting Air & Turbo Boost System Upgrades" Rev. 0 - This DCP upgrades the existing carbon steel piping and components in both the safety related and non-safety related portions of the 2A Emergency Diesel Generator (EDG) starting air and turbo boost systems.

New piping and valves (safety related and non-safety related) are stainless steel (TP 316L) to the extent practicable. These modifications upgrade the starting air and turbo boost systems to decrease their susceptibility to corrosion and improve their performance and reliability.

The modifications do not change the EDG starting requirements, loading on the EDG, or the required number of ED Gs assuming a single failure. There is no reduction in the margin of safety as defined in the bases for any Technical Specification. (SORC 96-47)

DCP 2EC-3353 Pkg. 1 "Replacement of LP Turbine Rotors with Fully Integral Design" Rev. 1 - This DCP involves removing and replacing the existing Low Pressure Turbine Rotors (Component I.D.

Nos. 2MSE6, 2MSE9, and 2MSE12) with new mono-block design rotors. The ruggedized rotor is a fully integral design and eliminates shrunk-on discs and key plates. All equipment is specified to meet or exceed material and design requirements. There is no reduction in the margin of safety as defined in the bases for any Technical Specification. (SORC 96-067) 2EE-3356, Pkg. 1 "#25 SW Pump Replacement" Rev. 1 - This DCP replaced the No. 25 Layne & Bowler Service Water pump with a new Johnston Pump Co. pump with a slightly different design. The 2SWE5 replacement Service Wate~ pump material is compatible with brackish river water, able to withstand the high silt flows, and designed for an NPSH at 76 feet river water level. The acceptance criteria is not being changed. There is no reduction in the margin of safety as defined in the bases for any Technical Specification.

EVALUATIONS DOCKET NO: 50-311 MONTH: JUNE 1996 UNIT NAME: SALEM2 CONTACT: R.HELLER TELEPHONE: 609-3 3 9-5162 2EC-3377, Pkg. 2 "Emergency Diesel Generator (EDG) Jacket Water Piping Modification" Rev. 0 - This DCP replaced and rerouted Jacket Water instrument piping/tubing and valves to reduce the vibration induced stresses to within acceptable limits.

To accomplish this, the system was tuned so that the significant system natural frequency was not resonant with dominant diesel generator vibrations. In addition, this DCP adds flexhose, tubing, and valve supports, and bleed valves to the instrument process lines. These modifications mitigate Jacket Water system instrument line vibrations, thereby increasing reliability and availability of the EDG.

These modifications do not affect the load on the EDG, the EDG output capacity, or the number of ED Gs that are available for service. There is no reduction in the margin of safety as defined in the bases for any Technical Specification. (SORC 96-020) 2EC-3384, Pkgs. 1-4 "SIG Primary Manway Bolting Replacement/Hole Restoration" Rev. 1 - This DCP upgrades the primary manway bolting on the Salem Unit 2 Steam Generators (SG). The installation of helical coil inserts in the SG primary manway bolt (stud) holes, and the use oflonger bolts with washers or studs and nuts with washers, ensures that reactor coolant system integrity continues to be maintained. Design, procurement, and installation activities associated with the helical coil inserts satisfy all of the requirements of ASME Code Case N-496, and load capacity assessments of the inserts, replacement bolts/studs, washers, and manway reinforcement area verify that these components have adequate structural capacity for both normal and abnormal operating conditions. There is no reduction in the margin of safety as defined in the bases for any Technical Specification. (SORC 96-058)

This DCP installs two new voltage sensing relays and an auxiliary relay to monitor the output voltage of the Emergency Diesel Generator (EDG) 2B. The setpoints of the new voltage sensing relays are at a higher voltage than the existing relays and will prohibit the EDG main circuit breaker from closing when the EDG output is less than the required voltage. The modifications improve the system operation, and will not prevent the EDG from operating in accordance with the Technical Specifications. There is no reduction in the margin of safety as defined in the basis for any Technical Specification. (SORC 96-045) 2EC-3389, Pkg. 1 "Penetration Area Blowout Panel Modification" Rev. 0 -

This DCP implements modifications which resolve the obstructions and degraded material conditions for the existing pressure relief path and, in-turn, preclude the possibility of compartment pressurization beyond the design basis due to the postulated pipe breaks. This is to be accomplished by (a) reevaluating the room pressurization's using smaller as-built vent area than that previously considered; and (b) repairing selected panels that have degraded conditions that would prevent them from functioning as intended. The proposed change package restores vent panels and their fasteners to a more reliable condition and improve reliability of the pressure relief function of these panels. There is no reduction in the margin of safety as defined in the bases for any Technical Specification. (SORC 96-053) 2EC-3390 Pkg. 1 "Annular Steel Column and Surface Channel Modifications" Rev. 0 - This DCP modifies annular steel columns in eight (8) locations by bracing the columns to the crane wall with W6 members. The surface mounted channels were modified by the addition of an angle bracket to reduce their span length. One light fixture was relocated to facilitate this

These modifications increase the margin of safety for the annular steel columns and surface mounted channels. There is no reduction in the margin of safety as defined in the bases for any Technical Specification.

(SORC 96-033) 2EC-3439, Pkg. 1 "Main Steam Drain Orifice Replacement Project" Rev. 0 -

This DCP removed the existing orifice coupling which regulated the steam flow from the above seat and below seat drain lines off of Turbine Trip Valve (2MS52), and replaced it with a new orifice coupling having a smaller bore, and installed a new strainer upstream of the orifice to prevent it from clogging. There is no reduction in the margin of safety as defined in the basis for any Technical Specification. (SORC 96-045) 2EC-3442, Pkg. 2 "Diesel Generator Crankcase Exhauster Piping Modification" Rev. 0 - This modification involved removal of Temporary Modifications and rerouting the Emergency Diesel Generator (EDG) crankcase exhaust vent piping in order to provide a permanent corrective modification. This modification reroutes the crankcase exhaust fumes at a location and elevation remote from the EDG Control Room Ventilation intake to prevent recirculation of the fumes.

Additionally, the piping was provided with tornado missile protection in accordance with Regulatory Guides 1.117 and 1.9. Nothing implemented by this DCP affected the ability to maintain at least two diesels operable during implementation and three diesels ultimately. There is no reduction in the margin of safety as defined in the bases for any Technical Specification. (SORC 96-050) 2EC-3456, Pkgs. 1 & 2 "Diesel Generator Air Compressor Setpoint Change" Rev. 0 Package 1 This modification replaced pressure control switches for Emergency Diesel Generator 2A starting and turbo-boost

The new switches are ASCO Model SAl 1D/THIOA21 which have an adjustable operating range of 13-300 PSJG. To eliminate abnormal setpoint drift between calibrations resulting from tubing-induced vibration, flexible hose were added between the metal tubing and the instrument ports.

Package 2 This DCP replaced pressure control switches for Emergency Diesel Generator (EDG) 2B starting and turbo-boost air compressors. The new switches are ASCO Model SAI 1D/THIOA21 which have an adjustable operating range of 13-300 PSIG. To eliminate abnormal setpoint drift between calibrations resulting from tubing-induced vibration, flexible hose were added between the metal tubing and the instrument ports.

Since these modification do not adversely affect the reliability of the starting and turbo-boost air systems, there is additional assurance that the EDGs will start and operate efficiently to mitigate accident consequences. There is no reduction in the margin of safety as defined in the bases for any Technical Specification. (SORC 96-050) 2EC-3467 Pkgs. I & 2 "Package I - Modification to SJl &2 Valve Disks, Package 2 - Modification to SJI 13 Valve Disks" Rev. 0 -

These modifications drill 1/8" weep holes in the valve disc (Pump side) of the Safety Injection Refuel Water Storage Tank to Charging Pump Valves 2SJ1 and 2SJ2 and the 2SJ114 side of the Safety Injection RHR to Charging Pump Valves 21 SJI 13 and 22SJ1 13. These modification will enhance the reliability of the system by providing internal relief paths for any fluid trapped in the valve bonnets, thus ensuring valve operability. There is no reduction in the margin of safety as defined in the bases for any Technical Specification. (SORC 96-041)

'EVALUATIONS DOCKET NO: 50-311 MONTH: JUNE 1996 UNIT NAME: SALEM2 CONTACT: R.HELLER TELEPHONE: 609-339-5162 2EC-3559, Pkg. 1 "Fuel Handling Building Auto Start" Rev. 0 - This DCP provides the required instrumentation and power to automatically initiate the Salem Unit 2 Fuel Handling Area Ventilation System exhaust fans on a high radiation signal.

This circuitry is parallel to the existing interlock between the radiation monitors and the Fuel Handling Ventilation dampers. This DCP also provides relay interlocks between the associated Radiation Monitors and the Fuel Handling Area Ventilation System Exhaust fans on a high radiation signal from either the 2R5 or the 2R9 radiation monitors.

This DCP is an enhancement to safety and is being performed to provide the capability required by the existing Tech Spec Surveillance Requirement 4.9.12.d.2. There is no reduction in the margin of safety as defined in the bases for any Technical Specification. (SORC 96-068)

2. Procedures and Revisions S2.CH-AD.CN-1144(Q) "Non-routine Unit 2 Steam Generator Fill and Chemical Feed" Rev. 0 - This procedure provides method for filling Unit 2 Steam Generators (SG) using Demineralized Water (DM) when Auxiliary Feedwater or Auxiliary Chemical Feed Systems are unavailable during Mode 6 or Mode Undefined. This method provides a location to safely add chemicals while filling SGs when Auxiliary Feedwater or Auxiliary Chemical Feed Systems are cleared and tagged otherwise unavailable. This method also allows filling the SGs with low dissolved oxygen content DM water. The introduction of hydrazine and ammonium hydroxide through the Generator Blowdown piping and valves will not threaten the Control Room habitability. A Chemistry technician, trained to deal with small chemical leaks, will be present continuously during the entire chemical addition. There is no reduction in the margin of safety as defined in the bases for any Technical Specification. (SORC 96-075)

3. Safety Evaluations S/E for 2EC-3224-0l "Spent Fuel Pool Rerack" Rev. 2 - This DCP installed the Holtec high density fuel storage racks in the Unit 2 spent fuel pool, thereby changing the facility as described in the UFSAR. Revisions 1 and 2 of this safety evaluation addressed the actual physical changes of this modification.

Research of regulatory documents has revealed that "emergency" full core offload capability is, in fact, not a licensing/regulatory requirement (see 10CFR53, Statements of Consideration dated 3/13/85). Revision 3 of this safety evaluation removes the limitation of Revision 2 of Unit 2 not entering Mode without resolution of the identified concerns. The fuel handling accident is the bounding accident of the Fuel Handling Building, and this revision demonstrates that the margin of safety for a fuel handling accident is not affected by this modification. This revision also makes the results of a loss of pool cooling event inconsequential by creating a condition where the worst previous result, boiling and its consequences, are eliminated.

There is no reduction in the margin of safety as defined in the bases for any Technical Specification. (SORC 96-076) 2 RCE8 960410132 "Acceptability of Stabilizer Pull Cord Remaining in 24 Steam Generator" Rev. 0 - As a result of the partial removal of tube Rl 1C27 for metallurgical examination, the remaining portion required stabilization and plugging.

During stabilization of tube Rl 1C27 in 24 Steam Generator, the polyethylene cord used to pull the stabilizer assembly into position became tangled during tube stabilizer installation and could not be removed. The cord is not a part of the Steam Generator design. This evaluation concludes there is no reduction in the margin of safety as defined in the basis for any Technical Specification.

(SORC 96-080)

. EVALUATIONS DOCKET NO: 50-311 MONTH: JUNE 1996 UNIT NAME: SALEM2 CONTACT: R.HELLER TELEPHONE: 609-3 39-5162 SIB for 2EC-3407-01 "Steam Trap Replacement Project" Rev. 1 - This modification changes the facility as described in the 'SAR.

UFSAR Sections 10.3.2.1 and Section 10.4.7.2.2 will be revised to reflect the addition of restricting orifices. Figure 10.3-lB will be revised to reflect the DCPs proposed for the steam trap replacements. This portion of the main steam and turbine drain piping system is non-safety related. There is no reduction in the margin of safety as defined in the bases for any Technical Specification. (SORC 96-085).

SIB for 2EE-0249-0l "Removal of Moisture Separator Reheater (MSR) Flow Elements" Rev. 0 - This DCP removes the following 12 flow instruments: 2FA447, 2FA572, 2FA574, 2FA692*,

2FA695, 2FA698*, 2FA704, 2FA706, 2FA708, 2FA710*,

2FA737* and 2FA2968*. The removals include the instrument, transmitter, associated wiring, process interface flange connections and welding a pipe cap over the pipe stub. These changes also include closing TMODS91-052 for 2FA710 and 92-019 for 2FA710 and 92-019 for 2FA447. All flanges have been verified to be either 3 inch or 4 inch "T" flanges as opposed to "in-line" flanges. There is no reduction in the margin of safety as defined in the bases for any Technical Specification. (SORC 96-085)

4. Temporary Modifications (TMOD)96-022 "Main Hoist Power Lifted Leads on the Cask Handling Crane" Rev. 0 - The purpose of this TMOD is to prevent inadvertent energization of the main hoist, which is currently damaged, and allow use of the auxiliary hoist.

There is no reduction in the margin of safety as defined in the basis for any Technical Specification. (SORC 96-086)

During the implementation of this DCP the Unit 2 Equipment Room, Unit 2 Data Logging Room, and Unit 2 Relay Room environments will remain within their design temperature limits. The Chilled Water system has no Technical Specification requirements during the plant conditions defined for this TMOD. SUMMARY

- UNIT 2 JUNE 1996 SALEM UNIT NO. 2 The Unit remained shutdown for the entire period. According to commitments from PSE&G and a subsequent confirmatory action letter from the NRC, the Unit will remain shutdown pending completion of the following actions:

  • Appropriately address long standing equipment reliability and operability issues
  • After the work is completed, conduct a restart readiness review to determine for ourselves the ability of each Unit to operate in a safe, event free manner
  • After the restart review, meet with the NRC and communicate the results of that review
