Inspection Report Nos. 50-272/89-17
This letter refers to the special safety inspection conducted to review the
Joss of Residual Heat Removal Systems (RHR) event on May 20, 1989 due to
inadvertent injection of nitrogen from one of the accumulators and your actions
to respond to this event.
The inspection was conducted by a team led by
Dr. P. K. Eapen of this office on May 22-26, 1989, at Salem Unit 1, Lower
Alloways Township, New Jersey.
A discussion of our findings was held by
Dr. Eapen with you at the conclusion of this inspection.
Areas examined during this inspection are described in the NRC Region I
Inspection Report which is enclosed with this letter. Within these areas, the
inspection consisted of selective examinations of procedures and representative
records, interviews with plant staff, and observations by the inspectors.
We note that certain operator actions during this event were inappropriate.
These inappropriate.actions included:
the initial operator error of unblocking
the wrong valve from the back panel, and the operator action to drain the
Reactor Coolant System (RCS) when the pressurizer level went off scale high
without realizing that the RCS may be nitrogen bound.
Further, the initial
engineering evaluation did not adequately address the event.
We also noted that the operator training you provided in response to NRC Generic
Letter 88-17, Loss of Decay Heat Removal, was beneficial to the operator while
venting the RHR System when* it was gas bound.
Based on the results of this inspection, it appears that two activities were
not conducte.d in full compliance with NRC requirements, as set forth in the
Notice of Violation, enclosed herewith as Appendix A.
These violations have
been categorized by severity_ 1eve1 in accordance with the "Genera 1 Statement of
Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C
(Enf6rcement Policy).
You are required to respond to this letter and in
preparing your responsi, you should follow the instructions in Appendix A .
