APPENDIX A NOTICE OF VIOLATION Public Service Electric and Gas Company Salem Nuclear Generating Station Unit 1 Docket No.
50-272 License No. DPR-70 As a result of the inspection conducted on May 22-26, 1989, and in accordance with 11General Statement of Policy and Procedure for NRC Enforcement Actions 11,
10 CFR 2, Appendix C (Enforcement Policy 1989), the following violations were identified:
A.
10 CFR 50.72 (b)(2) 11 Four-hour reports, 11 requires that the licensee shall notify the NRC within four hours of the occurrence of any event or condition that alone could have prevented the fulfillment of the safety function of structures or systems that are needed to remove residual heat..

.Cont.rary to the above, on May 20, 1989, when the residual heat removal capability was lost for about fifty minutes, the licensee did not notify the NRC within four hours.
- This is a Severity Level IV violation (Supplement I).
B.
Technical Specification 6.8.2 requires that surveillance and test activity procedures shall. be independently reviewed to make a determination as to whether the procedure contains a significant safety issue (SSI).
Additionally, Technical Specification 6.5.1.6 requires that the Station Operations Review Committee (SORC) shall review procedures that involve an SSI.
Contrary to the above, on May 20, 1989, Surveillance Procedure No.
. SP(0)4.0.5-V-SJ-6, 11 Inservice Testing - Valves - Safety Injection, 11 which involved an SSI, injection of nitrogen into the reactor coolant system, was not reviewed by SORC.
This is a Severity Level IV violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, Public Service Electric and Gas Company is hereby required to submit to this office within 30 days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved.
Where good cause is shown, consideration will be given to extending thi? response time.
