.. NOTICE. OF "VIOLATION Pufiltc Service Electric and Gas Company Docket No. 50~311 Based on the results of an NRC inspection conducted on July*l-31, 1980, it appears* that one of your activities was not conducted i"n full compliance with conditions of your.Facility License No. DPR-75 as indicated below. This item is an infraction.
Technical Specification 3.0.4 states, "Entry into an OPERATIONAL MODE or other specified applicability condition shall not be made unless*
the conditions of the Limiting Condition for Operation are met without
- reliance on provisions contained in the ACTION statements unless other~
wise excepted * "
Contrary to the above, the unit was heated up to Mode 3 on July 9, 1980 with the Containment Spray System and the steam-driven Auxiliary Feed-water Pump inoperable.
On July 14,1980, valves 21CS6 and 22CS6 were found tagged closed, rendering the Containment Spray System inoperable (Technical Specification 3.6.2.1}.
On July 13, 1980 valves 21MS45 and 23MS45 were found tagged closed, rendering the steam driven Auxiliary Feedwater Pump inoperable (Technical Specification 3.7.1.2). In each case, the ta.ggi ng predated the entry into Mode 3
