This supplemental report includes a summary of the results of the root cause analysis performed to determine the reasons for the failure of fuel assembly 1-024 and the subsequent discharge of nuclear fuel in~o the prtmary coolant of the Palisades reactor.

Art NRC Augmented Inspection Team (AIT). was dispatched to Palisades on July 8, 1993 to investigate Consumers Power Company's (CPCo) actions to address the fuel rod failure and unrelated personnel errors at Palisades.

The AIT issued the results of their investigation in Inspection R~port (IR) 93018.

By letter dated September 30, 1993,. we provided a detailed description of the root cause analysis and its conclusions.

This ~vent was originally reported in accordance with IOCFR50~73(a)(2)(ii).

No further supplements to this_LER are expected.
