PALISADES NUCLEAR PLANT - NOTIFICATION OF AN NRC AGE-RELATED DEGRADATION FOCUSED INSPECTION; AND REQUEST FOR INFORMATION 05000255/2013004

Dear Mr. Vitale:

On August 26, 2013, the NRC will begin the onsite portion of the age-related degradation focused inspection at your Palisades Nuclear Plant. This inspection will be performed in accordance with NRC inspection procedure (IP) 71152, Problem Identification and Resolution (PI&R).

In order to minimize the impact that the inspections have on the site and to ensure a productive inspection, we have enclosed a request for documents needed for the inspection. The documents have been divided into three groups.

  • The first group lists information necessary for our initial inspection scoping activities. This information should be available to the lead inspector no later than July 29, 2013. By August 5, 2013, the lead inspector will communicate the initial selected samples.
  • The second group of documents requested is those items needed to support our in-office preparation activities. This set of documents should be available at the Regional Office no later than August 15, 2013. This information should be separated by information request number, especially if provided electronically (e.g., folder with the information request number). During the in-office preparation activities, the inspectors may identify additional information needed to support the inspection. The lead inspector will provide a list of the additional information needed during the week of August 19, 2013.
  • The last group includes the additional information above as well as plant specific reference material. This information should be available to the inspector on August 26, 2013. It is also requested that corrective action documents and/or questions developed during the inspection be provided to the inspector as the documents are generated.

A. Vitale It is important that these documents be as complete as possible, in order to minimize the number of documents requested during the preparation week or during the onsite inspection.

If no activities were accomplished associated with a specific request, then provide a statement indicating that the request is not applicable.

This letter does not contain new or amended information collection requirements subject to the Paperwork Reduction Act of 1995 (44 U.S.C. 3501 et seq.). The NRC may not conduct or sponsor, and a person is not required to respond to, a request for information or an information collection requirement unless the requesting document displays a currently valid Office of Management and Budget Control Number.

Initial Selection of Components and Subcomponents The following list includes the initial selection of components and subcomponents for which Sections III.6 and IV.2 of this enclosure applies. During the onsite inspection, the inspectors may expand or reduce this selection.

1. Couplings in emergency diesel generators (EDGs), motors of emergency core cooling system pumps, feedwater pumps, and primary coolant pumps.
2. Belts in EDGs and its auxiliaries, and control room ventilation system.
3. O-rings in EDG fuel oil injection pumps.
4. Seals in control room ventilation system dampers, and main condenser.
5. Gaskets in EDG jacket water coolers, and component cooling water (CCW) heat exchangers.
6. EDG hoses.
7. Diaphragms in diaphragm-operated spray control valves of the pressurizer spray system, and flow-control valves of the auxiliary feedwater system.
8. Safety-related expansion joints.
9. Electrolytic capacitors in EDG governor control system and battery chargers.
10. Relays in service water (SW) common header crosstie isolation valves, primary containment isolation system logic, and primary coolant pump ground fault.
11. Radiation detectors found in the SW and CCW systems.
12. Valve actuators associated with SW common header crosstie isolation valves, and turbine stop valves.
13. Snubbers.
14. Molded case circuit breakers.
15. Printed circuit boards of battery chargers.

16. Integrated control system of turbine control system.
17. Surge arresters of offsite power transformers.

II. Information Requested By July 29, 2013:

1. Copy of the Q-list or equivalent list of safety-related structure, systems, and components (SSCs).
2. Copies of self-assessments related to Age Management Programs and age-related issues completed during the last three years and any associated corrective action program (CAP) documents.
3. If available, provide the list or document tracking SSCs known to be in service beyond vendor-recommended lifetime or known to not have vendor-recommended lifetime.
4. Provide a list of CAP documents (with a short description including the affected component and system) closed during the last three years associated with:

(a). Corrosion, oxidation, and rust; (b). Cavitation; rattling or popping sounds in piping; (c). Flow-accelerated corrosion and micro-biologically induced corrosion; (d). Impingement; (e). Wear; (f). Fatigue; (g). Embrittlement; (h). Cracking, blistering, spalling, and scaling; (i). Settlement, misalignment of components, and deformed structures; (j). Delamination and peeling; (k). Hardening; (l). Abnormal temperature; (m). Fracture; (n). Spurious electrical activity or faults; (o). Leakage; (p). Age-related degradation or aging; (q). Age Management Programs; (r). Service life or operating life; and (s). Shelf life.

5. If available, electronic copies of the current Technical Specifications, Technical Specifications Basis, Final Safety Analysis Report, and Technical Requirements Manual.

III. Information Requested By August 15, 2013:

1. Copies of the following implementing procedures:

(a). Preventative Maintenance (PM) Program; (b). Maintenance Rule Program; (c). Quality Assurance Program; (d). CAP Program; (e). Operating Experience Program; (f). Operability Determination Procedure; (g). Procedures associated with storage of important to safety and safety-related replacement items; and (h). Procedure that defines component critical and duty-cycle rankings (e.g.,

low-critical and low-duty-cycle classifications).

2. Provide a list of your Aging Management Programs and copies of the program basis documents for each.
3. Provide a list of replacement items in storage that have special storage requirements such as lubrication, periodic rotation, nitrogen blanks, desiccants, periodic inspections, and special environmental conditions. The list should specify the special storage requirement for each item.
4. Provide copies of the selected CAP documents and any associated attachment.
5. For the components and subcomponents listed in Section I, Initial Selection of Components and Subcomponents, of this enclosure:

(a). Provide a list of preventative maintenance (PM) activities deleted during the last three years including a brief description of the purpose of each deleted activity and reference number; (b). Provide a list of PM activities performed during the last three years including a brief description of the purpose of each activity and reference number; (c). Provide a list of work packages (e.g., work orders) associated with the replacement of these components and subcomponents during the last three years; and (d). Provide a list indicating the critical and duty-cycle ranking for these components and subcomponents.

1. For the selected SSCs known to be in service beyond vendor-recommended lifetime or known to not have vendor-recommended lifetime:

(a). Provide documentation of the last two completed PM activities.

(b). Provide the engineering assessment extending their service life, if available.

(c). Provide the vendor technical manuals, technical bulletins, and certificate of compliance for the selected SSCs and/or subcomponents. If available, electronic copies are preferred.

2. For the components and subcomponents listed in Section I of this enclosure:

(a). Provide the vendor technical manuals, technical bulletins, and certificate of compliance for the selected SSCs and/or subcomponents. If available, electronic copies are preferred.

(b). Provide documentation of the last two completed PM activities.

(c). Reference materials (make available if needed during onsite weeks) may include system health reports, maintenance records, procurement documents, plant procedures, and calculations associated with the selected SSCs and/or subcomponents.
