Initial Conditions:
Due to a plant transient, Control Rod M-4 is misaligned from its bank by 24 steps.
Unit 1 has been stabilized at 45% power for the past 16 hours1.851852e-4 days <br />0.00444 hours <br />2.645503e-5 weeks <br />6.088e-6 months <br />.
The crew entered AP/1/A/5500/14 (Rod Control Malfunction), and completed Enclosure 1 (Response to Dropped or Misaligned Rod) through Step 16.
IAE and RE has determined that rod realignment is permissible by moving the misaligned rod inward.
Initiating Cue:
The CRS has directed you to realign Control Rod M-4 with Bank D by performing Enclosure 1 (Response to Dropped or Misaligned Rod), of AP/1/A/5500/14 (Rod Control Malfunction) starting at Step 17.
IAE is standing by to assist in the realignment.
Task Standard:
The operator will correct the misalignment of Control Rod M-4 by performing Steps 17-22 and 45.a of Enclosure 1 (Response to Dropped or Misaligned Rod), of AP/1/A/5500/14 (Rod Control Malfunction).

Reset simulator to IC-37 (At power >50%/Rods in AUTO).
- 2.
Insert MALF-IRE010M4 (Stuck Rod [M-4 in Control Bank D]).
- 3.
Place Simulator in RUN.
- 4.
Insert MALF-DEH002A4 (Turbine Runback).
- 5.
Respond to plant via AP/1/A/5500/03 (Load Rejection).
- 6.
Ensure Control Rods moving inward with Control Rod M-4 not moving.
- 7.
When Control Rod M-4 is higher than the remainder of Control Bank such that DRPI reads 72 Steps, and the other Group D rods indicate 48 Steps, Freeze the Simulator, and REMOVE MALF-IRE010M4 (Stuck Rod [M-4 in Control Bank D]).
- 8.
Place Simulator in RUN, and stabilize the plant at 45%.
- 9.
Perform the actions of AP/1/A/5500/14 (Rod Control Malfunction), through Step 16 of (Response to Dropped or Misaligned Rod).

Freeze the Simulator OR
- 1.
Reset to IC-252 (February, 2012)
- 2.
Momentarily go to RUN to acknowledge Alarms then place Simulator in FREEZE.

- 16 (Step 21.f) Place the misaligned rod's group demand step counter to the value calculated in Step 21.e.
The operator places the Control Bank D Group 2 step counter to 69.
17 (Step 21.g) Consult with IAE to determine where the misaligned rods should be positioned to restore proper alignment.

IAE reports that Control Rod M-4 should be inserted such that Control Bank D Group 2 step counter indicates 49.

Initiating Cue:
The CRS has directed you to complete the Turbine OPC and Mechanical Overspeed Trip Test per PT/1/A/4250/004C (Turbine OPC and Mechanical Overspeed Trip Test), starting with Step 12.7.
Task Standard:
The operator will raise Turbine speed to OPC setpoint, and then raise speed until the Turbine Overspeed trip should be actuated. The operator recognizes the turbine has failed to trip at the expected setpoint and then manually trips the Turbine.

The operator reads the NOTEs and reviews Steps 12.11 - 12.14, and proceeds.
- 6 (Step 12.11) Depress "GO."
The operator presses the GO pushbutton on the Turbine Control Panel, and observes White status light is LIT.
7 (Step 12.12) Check Turbine starts increasing speed at selected rate.
The operator observes the Digital Turbine Speed indicator and determines that Turbine speed is rising at the expected rate.
8 (Step 12.13) IF demand speed of 1860 rpm is reached before OPC Controller actuates, perform one of the following:
(Step 12.13.1) Reduce Turbine speed to 1800 rpm (100%).
OR (Step 12.13.2) IF unable to reduce speed, have operator at Turbine Trip Lever trip Turbine.
The operator observes that Turbine Speed rises to 1860, and stops, and places and NA in this step.

The CRS has directed you to remove Pzr Heater Groups A, B and D from service per Enclosure 4.6 (Operation of Pzr Heaters) of OP/1/A/6100/003, and ensure that NC System pressure is being controlled normally at 2235 psig.
All outstanding R&Rs that may impact performance of Enclosure 4.6 have been evaluated.

Initial Conditions:
Unit 1 is operating at 100% power.
AP/1/A/5500/22 (Loss of VI) has been implemented due to decreasing VI Pressure.
VI Pressure is holding at 75 psig but the leak has not been isolated.
Several other operators have been dispatched for various tasks in response to the loss of VI.
Initiating Cue:
The CRS directs you to bypass the VI dryers and isolate VS per (VI Dryer and VI to VS System Isolation) of AP/1/A/5500/22 (Loss of VI) using the copy of the procedure located beside Service Bldg Lube Oil Station door.
Task Standard:
The operator will fail open the Dryer Bypass Valve when it is recognized that the valve has failed Closed, and then close the A, B, and C VI Dryers Inlet and Outlet Isolation valves.
