1.0 INTRODUCTION

Electric Power Research Institute (EPRI) Report 1025286, Seismic Walkdown Guidancefor Resolution of Fukushima Near-Term Task Force Recommendation 2.3.' Seismic, was issued in 3une 2012. This document provides guidance and procedures to perform seismic walkdowns as required by the U.S. Nuclear Regulatory Commission's (NRC's) 50.54(0 letter regarding Near-Term Task Force (NTTF) Recommendation 2.3: Seismic. The EPRI guidance covers selection of personnel; selection of a sample of structures, systems, and components (SSCs) that represent diversity of component types and ensures inclusion of components from critical systems/functions; conduct of the walkdowns; evaluation of potentially adverse conditions against the plant seismic licensing basis; peer review;.Individual Plant Examination of External Events (IPEEE) vulnerabilities; and reporting requirements. It was intended that all U.S. nuclear power plants utilize this guidance document in meeting the requirements of the NRC 50.54(f) letter.

Duke Energy (Duke) contracted with the Shaw Group (Shaw) / ARES Corporation (ARES) Team to perform the NTTF 2.3 peer review at the Catawba Nuclear Station (CNS). This report documents that peer review.

2.0 SCOPE The scope of this effort was to perform the NTTF 2.3 Seismic Peer Review at CNS, in accordance with the guidelines in Section 6, Peer Review, of EPRI 1025286. The peer review is to be documented in a submittal report as discussed in Section 8, Submittal Report, of EPRI 1025286. It is intended that the information contained herein will be utilized by Duke as part of its overall NTTF 2.3 submittal report to be delivered to the NRC in November 2012.

Per Section 6 of EPRI 1025286, the peer review should cover the following:

  • Review the selection of the SSCs included on the Seismic Walkdown Equipment List (SWEL).

" Review a sample of the checklists prepared for the seismic walkdowns and area walk-bys.

" Review the licensing basis evaluations.

  • Review the decisions for entering the potentially adverse conditions in the Corrective Action Program (CAP) process.
  • Review the submittal report.

" Summarize the results of the peer review process.

3.0 METHODOLOGY The Shaw/ARES methodology conforms to the guidance in Section 6 of EPRI 1025286. The Peer Review Team consisted of three individuals, all of whom have seismic engineering experience as it hv Shiawo( @ 9 Page I CNC-1206.03-00-0203 Rev 0 Attachment 6A Page 5 of 37

H. Thomas Baumgardner, Duke Energy Consultant. Mr. Baumgardner has 45 years of experience in fossil and nuclear power plant operations experience. Mr. Baumgardner was Senior Reactor Operator at CNS the late 1970s to the mid-I 990s. Following this, he was a supervisor and writer for the CNS Emergency and Abnormal Operating Procedures until his retirement in 2010. Mr. Baumgardner participated in the peer review of the SWEL.

5.0 SELECTION OF THE SSCs INCLUDED ON THE SWEL Guidance on development of the SWEL is provided in Section 3 of EPRI 1025286. Two SWELs are prepared: SWEL 1, a sample of items to safely shut down the reactor and maintain containment integrity; and SWEL 2, spent fuel pool related items. SWEL I is expected to contain 90-120 items for each unit. The CNS SWEL I contains 119 items for Unit I and 113 items for Unit 2. This satisfies the expectation from the EPRI guidance.

The development of the CNS SWEL is documented in a Catawba calculation package, CNC-1206.03-00-0202, 10CFR50.540')Recommendation 2.3 Near-Term Task Force (NTTF)Seismic Walkdowns, Revision 0. The calculation discusses the development of the seismic walkdown equipment list (SWEL 1) and the seismic walkdown spent fuel pool equipment list (SWEL 2). An in-process draft of the calculation was provided to the peer reviewers at the CNS site visit September 25-27, 2012. The final calculation and SWEL lists for each unit were provided to the peer reviewers on November 7, 2012.

The SWEL was originated by Mr. Thomas Leitch, Senior Engineer, CNS Design Engineering, and checked by Mr. Raymond Fung, Senior Engineer, CNS Design Engineering. Messrs. Leitch and Fung have extensive knowledge of CNS design, licensing and operation. Mr. Robert Pryce, a former Senior Reactor Operator, provided operations input. Mr. Baumgardner performed a peer review of the SWEL for operations as part of the Peer Review Team. Mr. Baumgardner was a Senior Reactor Operator at CNS and an AP/EOP Procedure Supervisor. Mr. Baumgardner's peer review of the SWEL has been documented in an email attached to the calculation (Attachment 10).

CNS started with the IPEEE Equipment List as the base list. This'list is documented in CNS Calculation CNC- 1535.00-00-0005, Revision 3, Seismic Capacity.Evaluationsfor 1PEEE andEPRI Margins Study.

Using this list is intended to satisfy Screens 1, 2 and 3 of the EPRI guidance document.

SWEL I was developed by selecting a sample of equipment from the base list. SWEL 2 was developed separately as described in Calculation CNC-1206.03-00-0202. The calculation includes separate attachments for the CNS Units 1 and 2 SWEL I and SWEL.2 lists.

The SWIEL 1 lists comply with the four screens in the EPRI guidance:

Screen 1: Seismic Category I. Because the non-safety (therefore, non-Category 1) equipment is identified, this screen was satisfied in the sample by not including the non-safety equipment.

Calculation CNC-1206.03-00-0202 contains a section on the development of the SWEL 2. There are two base lists developed for each unit: one for spent fuel cooling system components, the other for components that could cause rapid drain down. The base lists are in Attachment 6. The rationale for identifying the components for each list is provided in the calculation. There were 11 spent fuel cooling components identified for Unit 1, of which five were put on the SWEL 2 in Attachment 7, and ten for Unit 2, of which four were put on the SWEL 2. There were 16 rapid drain components identified for Unit 1, of which four were put on the SWEL 2, and 18 for Unit 2, of which five were put on the SWEL 2. The calculation has a discussion of why some of the base list components were not added to the SWEL 2. It is concluded that the SWEL 2 conforms to the EPRI guidance.

A preliminary peer review checklist was developed with specific comments to be addressed. This checklist is included as Appendix A to this report. The checklist and comments were discussed with Messrs. Thomas Leitch and Robert Pryce at the site exit meeting. The final calculation was reviewed to see if the comments were resolved. All of the comments are considered resolved. The final checklist is included as Appendix B of this report.

6.0 SEISMIC WALKDOWNS AND AREA WALK-BYS The equipment items and areas to be walked down were specified in the SWEL provided by Duke (CNC-1206.03-00-0202). The walkdowns consist of two parts: equipment-specific seismic walkdowns and area walk-bys. The specific instructions for each part are delineated in EPRI 1025286.

The seismic walkdown results are documented in Shaw /ARES Reports 1457690303-R-M-0000I and

-00002 for Units I and 2, respectively. Appendix A of the reports contains the filled out SWCs and AWCs. Appendix B of the reports contains a listing of the potentially adverse seismic conditions reported to Duke Energy for licensing basis review. Appendix C of the reports contains a listing of items that were inaccessible or had concealed anchorage. The evaluation of these items will be completed by Duke Energy.

Seismic walkdowns of specific items on the SWEL (SWEL I plus SWEL 2) focused on identifying adverse anchorage conditions, adverse seismic interactions, and other adverse seismic conditions that could challenge the seismic adequacy of a SWEL item.

Anchorage was examined for degraded, nonconforming or unanalyzed conditions. This included visual inspection of the anchorage and verification of anchorage condition. The visual inspections looked for bent, broken, missing or loose hardware; corrosion that is more than mild surface oxidation; visible cracks in the concrete near anchors; and other potentially adverse seismic conditions. This did not apply to line-mounted items.

Anchorage configuration was verified to be consistent with the existing plant documentation for a portion of the equipment with anchorage. The anchorage configuration verification must be done for at Shaw° I~ Page 5 CNC-1206.03-00-0203 Rev 0 Attachment 6A Page 9 of 37

NTTF 2.3 SEISMIC PEER REVIEW REPORT Report_No. 1457690303-R-M-00003, Rev. 0 CATAWBA NUCLEAR STATION UNITS I AND 2 November 2012 Interviews of walkdown personnel were jointly performed by the Duke, ARES, and Shaw members of the Peer Review Team. Personnel were interviewed as teams (two at a time) to assess their working synergy as well as individual capabilities/knowledge. Mr. Mark Eli was verified to have attended the EPRI NTTF 2.3 Seismic Walkdown Training Course (Mr. Eli is also a SQUG Seismic Capability Engineer). Messrs. North, Hanna, Rubright, Fazio, Berdos, Pappas, and Adam were verified to have received the EPRI NTTF 2.3 Seismic Walkdown Training Course from an individual (Mr. Mark Eli) who had taken the EPRI course in person. Discussion provided positive indication that the walkdown personnel had adequate experience and training to perform walkdown and walk-by activities in compliance with the EPRI Seismic Walkdown.Guidance. As teams, they displayed knowledge of the primary objectives of the walkdowns, appropriate levels of dialog between themselves to reach common agreement without excessive discussion, and adequate objectivity in identification of significant discrepancies between as-designed and as-found conditions. Team member qualifications are included in the unit-specific walkdown reports.

In-process review of the walkdown documentation packages verified sufficient information included to allow ready identification and location of in-situ SWEL components. Appropriate vendor drawings, anchorage details, calculations and/or license definitions or IPEEE documentation were included for the components specified for detailed inspection of anchorage provisions. A few inconsistencies were noted between document identifiers in the checklist entries and the package contents; however, these were easily resolved.

The Peer Review Team reviewed a number of in-process Area Walkdown Checklists (AWCs) and Seismic Walkdown Checklists (SWCs). The reviews are summarized in Appendix C. Initial Peer Review activity related to walkdown implementation concentrated on personal interviews and discussions with the walkdown teams, individually and as a group. Interaction with individual teams relied on posing hypothetical situations related to potential component configurations and assessing responses related to completion of the checklists. Group meetings, some including participation of Duke Energy personnel, addressed more generic issues such as how to deal with "non-50%" component anchorages, including obstructed views of anchorage provisions and cabinet interior conditions.

Clarification was provided on SWC questions 2, 3, 4 and 6 to address general as-found conditions without requiring design basis comparisons.

It was noted that observations/assessments are accurate but documentation is terse. Instances requiring

,team deliberation, drawing on specialized knowledge gained through seismic engineering experience, and/or follow-up evaluation activity couldlbenefit from a brief explanation and/or photos in the checklists. As the intent is not to include hard .copy in the final report, references (drawings, calculations, etc.) used in the walkdown should be explicitly identified in the walkdown packages.

Significant discussion centered on documentation ofjustifications for assessing in-situ conditions as acceptable with respect to seismic interaction considerations. Clarification was provided that informed judgment of SQUG/EPRI-trained individuals does not require explicit documentation, but that ambiguous or unusual conditions would be documented through written notes and/or the use of photographs.

8.0 DECISIONS ON ENTERING POTENTIALLY ADVERSE SEISMIC CONDITIONS INTO THE CAP PROCESS All of the potentially adverse seismic conditions identified by the seismic walkdown teams were entered in the CAP for further evaluation: The Peer Review Team review of the seismic walkdowns determined that the identifications of adverse seismic conditions were conservatively made. Thus, the decision to enter all of them into the CAP was likewise conservative.

The licensing basis reviews determined none of the potentially adverse seismic conditions violated the CNS licensing basis. Therefore, it was not necessary to perform any extent of condition evaluations.

There were a number of enhancements identified as a result of the walkdowns, which were determined to improve the seismic condition of the plant. Work orders were assigned for implementation of the enhancements.

The Peer Review Team concludes that the decisions on entering potentially adverse conditions in the CAP process were in accordance with the EPRI guidance.

9.0 SUBMITTAL REPORT The Peer Review Team reviewed a draft of the submittal report for CNS Unit 1 provided by Mr. Thomas Leitch on November 8, 2012. The report contained the required sections and discussions. Comments on the submittal were provided to Mr. Leitch on November 13, 2012. Updated submittal drafts for Unit I and Unit 2 were provided to the Peer Review Team on November 15, 2012. Further comments were provided on November 17, 2012.

The most significant comment is that Table 4-1 of the Unit I submittal, which contains the CAP dispositions for the potentially adverse conditions, contains one item (Item 28) that says current licensing basis is not met. This conflicts with the statement in Section 5 that all of the items were determined to meet the current licensing basis. In Table 4-1 of the Unit 2 submittal, this item is described as meeting the licensing basis. It is noted that this item is one of the items for which the Peer Review Team commented on the licensing basis review documentation not being sufficient to show the licensing basis was met.

Another significant comment is that Section 7, covering the Peer Review, needs to state that there are still comments outstanding.

It is recommended that the submittal include a discussion of the adequacy of CNS license monitoring and maintenance procedures in light of the findings of the walkdowns and licensing basis reviews, as this is one of the purposes of the NRC request stated in Section 1 of the EPRI guidance.

1.0 INTRODUCTION

Duke Energy (Duke) contracted with the Shaw Group (Shaw) / ARES Corporation (ARES) Team to perform the NTTF 2.3 seismic walkdowns at the Catawba Nuclear Station. Shaw performed the walkdowns on the accessible components. Duke personnel completed the walkdowns on the remaining components by the end of the IEOC20 refueling outage.

2.0 SCOPE The Duke portion of this effort was to complete the NTTF 2.3 seismic walkdowns and walkbys at the Catawba Nuclear Station Unit 1 for the eleven AWC's and sixteen SWC's which were inaccessible during the time the Shaw Group/Ares Corporation were on site.

3.0 METHODOLOGY The Duke walkdowns were performed using the methodology described in section 3.0 of the Shaw/ARES Report No. 1457690303-R-M-00001 (rev 2) that is part of this attachment.

4.0 PERSONNEL Walkdowns were performed by two person teams. The team members were T. R. Leitch, R. C. Fung, and J. D. Kennedy. All persons were qualified as SWEs in accordance with the requirements contained in Section 2, Personnel Qualification,of EPRI 1025286. Personnel qualifications are summarized in Table 2-1 of the submittal report.

5.0 WALKDOWN EXECUTION The Duke Team performed the seismic walkdowns and area walk-bys between November 1, 2012 and December 16, 2012, in accordance with the guidance provided in EPRI 1025286. The walkdowns and walk-bys were accomplished in the following manner:

  • Duke Team personnel reviewed the SWELs to become familiar with the extent of the walkdowns to be performed and the equipment to be walked down. This included SWEL 1, which includes items involved in safely shutting down the reactor and maintaining containment integrity; and SWEL 2, consisting of spent fuel pool related items.
  • Duke personnel:collected copies of required design information such as calculations, drawings, specifications, manuals, and Individual Plant Examination of External Events (IPEEE) documentation. The IPEEE documentation was not part of the plant licensing basis, but was useful for clarifying field conditions.
  • Walkdown packages for the equipment to be inspected, consisting of Seismic Walkdown Checklists (SWCs), Area Walk-By Checklists (AWCs), and supporting documentation were assembled.

Each team filled out the checklists, took notes, and discussed the observations. The Duke engineers took photographs to help document the results. Questions and uncertainties were recorded for follow-up action.

" As the project progressed, the SWCs and AWCs were updated as needed. The final checklists were signed by the Walkdown Team members. Supporting documentation and photographs, where needed to explain conditions reported on the checklists, were attached to the checklists.

7.0 RESULTS Duke personnel walked down 11 SWEL 1 items, and conducted 8 walkbys associated with these items.

Five SWEL 2 items were walked down, and 3 area walkbys associated with these items were performed.

Anchorage was verified on 4 of the 4 anchored items walked down by Duke. The detailed list of the walkdowns and walkbys, as documented on the SWCs and AWCs, is contained in Duke Appendix A.

Four new potentially adverse seismic conditions were identified which required licensing basis reviews.

Two involved seismic interactions and one involved hardware issues. All potentially adverse seismic conditions are identified in Duke Appendix B. Duke has entered each potentially adverse seismic condition into its corrective action program using PIPs. The NRC 50.54(f) letter and EPRI 1025286 requires the submittal report contain a description of how each potential adverse seismic condition is being addressed and its current status. Duke will provide this information in the submittal report.

Inaccessible items and items with concealed anchorage that were identified in Appendix C of Attachment 5A have their final status provided in Duke Appendix C.

The box was originally fastened to a piece of unistrut that goes to an embedment in the wall. These fasteners have come loose. Noted I VP7A - First tubing clamp off operator is loose- see pictures. This is a VI tubing clamp on the line attached to 1V13690. Tubing is still supported by clamp. Wrote work request 01075428 to tighten/repair.

I VY I7A - 4 inch VY (H2 sample and purge) pipe elbow is close to a Nuclear Safety Related cable junction box. Pipe seems rigid, and cable junction box could tolerate some contact, as it can flex somewhat. There is approx. 'A" clear currently. There is some slack in the cables, and the junction box is mounted on 4" electray that is mounted on unistrut.

This electric junction box assembly is not very massive, and seismic movement would be minimal due to stiffness of unistrut support. The piping is restrained in the lateral direction by two supports , I-A-VY-4008 and I-A-VY-4234, so lateral seismic motion of the piping should be negligible. See Photo.

Also noted that minimum 2" NV system clearancestandardbetween 2 inch NV line and unistrutsupporting a small electray is not met. The NV line is on CN- 149 1-NV.00- 117, elev 562'-9", and is almost directly above the pulsation dampener.(Approx Az 277, rad 59-4"). This is not a QA 1 line, so this 2" clearance is a good practice ratherthan a requirement. This clearancecan be achieved by removing enough materialfrom the unistrut to achieve the system clearance. I SSPSA anchorage was visible when cabinet was opened, and one other cabinet (lELCCO058) had anchorage that was visible when cabinet was closed. No other cabinets in the walk-by area were available to be opened. Anchors that were visible were free of potentially adverse seismic conditions.(see photo)

The HVAC Ducting and supports were in excellent condition. There are only a few cable /conduit raceways, they are not overfilled and are adequately supported.

Area was very neat and clean. Noted 4 hand held fire extinguishers hanging on hooks fairly low on columns in the area.

These are not likely to come off hooks during event, because the vertical component of seismic acceleration in this area is less.than Ig. Also, no significant spray targets were noted in the immediate vicinity of these devices.

A rigid cable tray support with approximately 3/8" clearance between the support and a liftinglug on IETXF was noted. (seephoto) This is acceptable by comparison, because rigid cabinets in this area will experience a lateral seismic movement of approx. 0.1 inches, and the transformer is at least as rigid asthese cabinets. Similarly, the cable tray support frame.is rigid, andseismicmovements. of approx 0.1 inches are expected. Therefore this clearance should be adequate to prevent interaction.
