READ TO OPERATOR DIRECTION TO TRAINEE:

I will explain the initial conditions, and state the task to be performed. All control room steps shall be performed for this JPM, including any required communications. I will provide initiating cues and reports on other actions when directed by you. Ensure you indicate to me when you understand your assigned task. To indicate that you have completed your assigned task return the handout sheet I provided you.

INITIAL CONDITIONS:

Unit #1 dropped a Control Rod in Group 6 approximately one hour ago and the unit is stable at 52% RTP.

AP/i, Unit Runback, has been completed.

The problem with the Control Rod has been repaired.

Systems Duty Engineer has been notified and no special maneuvering instructions are required. .15 (Recovery Of Dropped/Misaligned Safety Or Regulating Control Rod with Diamond In Automatic) of OP/I /A/1i05/0i9 has been completed up to step 2.

INITIATING CUE:

The Control Room SRO directs you to recover the dropped rod using an Automatic Latch beginning at step 2.1 of Enclosure 4.15 of OP/I/A/I105/019.

STEP 1: Step 2.1 SAT WHILE CRDs are moving, monitor the following indications:

  • Appropriate ranged NIs UNSAT
  • Startup Rate (if applicable)
  • Neutron error (if applicable)
  • RCS temperature (if applicable)

STANDARD: Candidate monitors the above indications on the OAC and control board.

COMMENTS:

STEP 2: Step 2.2 CRITICAL STEP Select affected rod as follows: (R.M.)

  • Ensure GROUP SELECT SWITCH to desired group. SAT
  • Ensure SINGLE SELECT SWITCH to desired rod.
  • Ensure RUN is ON.

UNSAT

  • Verify AUTO on Diamond is ON.
  • Verify MANUAL on Diamond is flashing.

STANDARD: *GROUP SELECT SWITCH is selected to group 6.

  • SINGLE SELECT SWITCH is selected to rod 6.

Ensure RUN is ON.

Verify AUTO on Diamond is ON.

Verify MANUAL on Diamond is flashing.

COMMENTS:

CRO-108 Page 6 of 10 STEP 3: Step 2.3 If affected rod is fully inserted and Manual Latch and P1 Alignement SAT desired, perform the following:

UNSAT STANDARD: Determines Manual Latch is NOT desired.

COMMENTS:

NOTE: When LATCH AUTO is selected, the following automatically occurs: *CRITICAL STEP

  • With ICS in AUTO, CRD TRAVEL lights will only respond to the controlling group when Auto Latching.
  • Rod inserts to latch rod SAT
  • Rod withdraws to test for latching
  • Rod inserts
  • RPI resets UNSAT STEP 4: Step 2.4 IF affected rod is fully inserted, perform Auto Latch and P1 Alignment, as follows: (R.M.)
  • Select LATCH AUTO.
  • WHEN Auto Latch is complete,

> Verify LATCH AUTO is OFF.

> Ensure desired rod API/RPI indications agree. (P1 Panel)

  • Select FAULT RESET.

STANDARD: *Depress the LATCH AUTO pushbutton on the Diamond.

WHEN Auto Latch is complete, Verify LATCH AUTO is OFF by observing LATCH AUTO light on the Diamond is NOT lit.

Use the toggle switch located on the P1 panel to verify Group 6 Rod 6 API!RPI indications agree.

  • Depress FAULT RESET pushbutton on the Diamond.

COMMENTS:

CR0-I 08 Page 7 of 10 CAUTION: If rod is known to have been dropped/misaligned for less than 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />, rod *CRITICAL STEP can be withdrawn at 30 in./min. (RUN Speed). {1} (R.M.)

If rod has been dropped/misaligned for greater than 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />, rod can be withdrawn in SAT 10% withdrawal increments spaced 30 mi apart at 30 in./min. (RUN Speed). {1} (R.M.)

NOTE: Controlling CRD Group will maintain Rx Power constant.

UNSAT Affected rod is considered recovered when rod position is group average and Manual light is no longer flashing.

STEP 5: Step 2.5 Perform the following until affected rod is realigned with group: (R.M.)

  • Begin withdraw of affected rod.
  • As required, stop withdrawal of affected rod.
  • IF affected rod is in Group with OUT LIMIT, continue rod withdrawal until affected rod is fully withdrawn.
  • IF affected rod is in Group NOT at OUT LIMIT, continue rod withdrawal until affected rod aligned with group.

STANDARD: *Begin withdrawal of the affected rod using the Joy Stick on the Diamond.

NOTE: At 40% withdrawn, TIMER 3 will automatically fire to drop Group 4 Rod 6.

COMMENTS:

ALTERNATE PATH STEP 6: Manually trip the reactor CRITICAL STEP SAT STANDARD: The candidate recognizes the second control rod inserting and manually trips the reactor by depressing the Reactor Trip pushbutton and verifies that the reactor is shutdown.

This action is required by OMP 1-18 Attachment A (Licensed Operator UNSAT Memory Items) which states:

Initiate a manual reactor trip if any of the following conditions exist:

OP/i/l 105/014 Enclosure 4.9 has been completed up to step 3.3.

INITIATING CUE:

The Control Room SRO directs you to remove ES Subsystem 2 from service per OP/1/A/1105/014 Enclosure 4.9 beginning with step 3.3.

INITIATING CUES The Control Room SRO directs you to perform Enclosure 5.12, ECCS Suction Swap to RBES to align HPI/LPI piggyback mode.

STEP 1: Step 1 CRITICAL STEP Start:

_1ALPI PUMP SAT lB LPI PUMP UNSAT STANDARD: Candidate locates the controls for 1A and 18 LPI pumps on 1UB2 and determines that 1A and 1 B LPI pumps are off by observing the white trip light illuminated and red close lights OFF.

Candidate rotates control switch to the CLOSE position for 1A (Critical) and I B (NOT Critical) LPI pumps. The red CLOSE lights are observed to be on and the white TRIP light is observed to be off.

Candidate observes the 1A LPIP amps are 20 amps and stable.

Candidate determines the lB LPIP did not start by no amps and the white TRIP light is still illuminated.

Candidate continues to Step 1 RNO COMMENTS:

STEP 2: Step 1 RNO SAT

1. IF NO LPI pumps are operating, THEN GO TO step 85.
2. IF 1C LPI PUMP is operating, THEN GO TO step 87 UNSAT STANDARD: Candidate determines that neither of the above are satisfied and continues to step 2.

COMMENTS:

SAT LPI FLOW TRAIN A olus LPI FLOW TRAIN B 3400 gpm Only one LPI header is operating, AND flow in that header is 2900 gpm UNSAT STANDARD: Candidate locates LPI Flow Train A and B flow meters on 1 UB1.

Observes flow in both headers to be 0 gpm.

Candidate refers to Step 2 RNO.

COMMENTS:

STEP 4: Step 2 RNO GO TO Step 4.

SAT STANDARD: Candidate goes to Step 4.

UNSAT COMMENTS:

STEP 5: Step 4 Verify three HPI pumps operating.

SAT STANDARD: Candidate locates HPI pump controls on 1 UB1 and determines that all three HPI pumps are operating by observing the red ON lights illuminated and HPIP motor amps are stable. UNSAT Candidate continues to Step 5.

COMMENTS:

CRO-306 Page 7 of 14 STEP 6: Step 5 CRITICAL STEP Stop lB HPI PUMP.

SAT STANDARD: Candidate locates the pump switch for the I B HPI Pump on 1 UB1 and stops the pump by rotating the switch to the OFF position.

Candidate verifies the red ON light goes out, the green OFF light is UNSAT illuminated, and motor amps go to zero.

NOTE: Candidate may throttle HPI per rule 6 guidance (<475 gpm) if flow in the A HPI Header is > 475 gpm.

Candidate continues to Step 6.

COMMENTS:

STEP 7: Step 6 CRITICAL STEP Simultaneously open:

SAT 1LP-15 1LP16 STANDARD: Candidate locates 1LP-15 and 1LP-16 on 1UB2 and opens both valves UNSAT simultaneously by taking each switch to the open position. Verify that the closed green light goes off and the red open light illuminates for each valve.

Candidate continues to Step 7.

COMMENTS:

CRO-306 Page 8 of 14 ALTERNATE PATH STEP 8: Step 7 Verify two LPI pumps operating. SAT STANDARD: Candidate determines that ONLY the 1A LPI pump is operating by observing that the 1A LPIP has amps in indicates the breaker is closed UNSAT while the 1 B LPIP has no amps in the white TRIPPED light is illuminated..

Candidate continues to Step 7 RNO which directs the candidate to Step 10.

COMMENTS:

STEP 9: Step 10 Maximize total LPI flow < 3100 gpm by throttling HPI flow.

SAT STANDARD: Candidate determines that there is 0 gpm LPI flow and since RCS pressure is above the shutoff head for the LPI pumps, LPI cannot inject and is therefore maximized. UNSAT COMMENTS:

STEP 10: Step 11 CRITICAL STEP Limit total HPI flow to < 750 gpm including seal injection SAT STANDARD: Candidate sums HPI header flows and seal injection flow and determines that total HPI flow is > 750 gpm and throttles 1 HP-26 and/or 1 HP-27 to reduce total HPI flow to <750. UNSAT As left flows: A header Seal Inj.

B header COMMENTS:

CRO-306 Page 9 of 14 STEP 11: Step 12 Place LDST LEVEL INTERLOCK switch in DISABLE.

SAT STANDARD: Candidate places LDST LEVEL INTERLOCK switch located on 1 UB1 to DISABLE by taking the switch to the DISABLE position.
