.4 O*NSITE GROUND/SURFACE WATER MONITORING A,QUESTIONNAIRE Surry Power Station

  • Onsite RadiologicalEffluent/REMP Monitoring Program Phase I !Near term response)

Yes No

1. Does the licensee have radioactive groundwater monitoring wells onsite?

[] C If YES: How many wells: One Where ar6Tthey located (e.g., distributed around/throughout the site, in a particularregion of the site and/or near particular buildings/structures, etc.)

(a!)) within the Protected Area C

(b.) within the Radiologically Restricted Area C

(c.) within the owner-controlled area C

(d.Jt what frequency does the licensee sample/analyze the wells Quarterly (eW.hfor what radionuclides does the licensee monitor Gamma emitters (gamma Spec) (

C If Yes - at what MDA I to 60 pCi&L Tritium

[] C If Yes - at what MDA 2,000 pCi/L Gross Beta C []

If Yes - at what MDA Other: C [

If Yes - at what MDA

2. If the licensee does NOT have an onsite radioactive groundwater monitoring program:

(a.) Does the licensee plan to implement a groundwater monitoring program?. C C If Yes, when and to what extent (b.) Does the licensee plan to take other measures to assure they can identify radioactive groundwater contamination C C

3. Does the licensee have a french drain system surrounding the main reactor facility and auxiliary structures ? C (a.) is the system analyzed for radionuclides?

C (b.),at what frequency does the licensee sample/analyze the wells Weekly

/5

(c)for what radionuclides does the licensee monitor Gamma emitters (gamma Spec)

C If Yes - at what MDA 5e-7 itCi/ml Tritium C

If Yes - at what MDA le-5 itCi/ml Gross Beta If Yes - at what MDA

4. Does*thý licensee have a surveillance program to periodically:

(a.) walkdown outside areas around the site to look for potential leaks

' d spills?

M C (b.) prebsurize buried radwaste lines to evaluate structural integrity and evaiuate potential for leaks and spills? C

5. Does'the licensee perform any other onsite monitoring (e.g. soil sampling) to identify unexpected radioactive releases C
6. Does the licensee's radioactive liquid discharge line traverse any non-licensee owned areas (e.g., it is on a right-of-way surrounded by C private properties)?

Yes No

7. If the licensee has a discharge pipe that runs underground or any underground piping that carries radioactive liquids, does the licensee C M perform monitoring along the discharge pathway to identify potential leakage.

If YES, How frequently is the sampling performed:

Phase II (Longer Term Response)

Yes No

8. Historical Onsite Radioactive Contamination:

(a.) Does the licensee have any history of radioactive spills and/or leaks C

Are they documented in 10 CFR 50.75g file?

C (b.) Has tlhe licensee identified onsite radioactive groundwater M]

contamination? C If YES: ,

When was it identified - IF known:

Dltes:

LER/Abnormal Event Report/Condition Report Nos:

___(If available)

To whýa"t extent - IF known [square footage, estimated ground depth of the

W4. U.ý contamination, estimated quantity (volume / concentration), etc.]

Has the cqntamination moved outside the Restricted Area or C C the owner-controlled area mments: Continuation of question 3c: Gross alpha le-7 piCi/ml MDA, Sr-89/90 5e-8 pCi/ml and Fe-55 le-6 pCi/mi MDA.

iestion #1: One onsite monitoring location, REMP sample location Surry Station iestion #3: Referring to Storm Drains. Strom Drains are fed by turbine, containment & fuel buildings sub-surface drains. Part of Rad Effluent program.

iestion #4a: Considered yes by Ops & Eng because it is part of an Operators watch station to be observant for leaks &.spills.

iestion #4b: Per Eng., radwaste lines are not code piping that requires testing.
