Inservice Inspection Summary Report for RF014 at PNPS Ptease find attached the IS1 Summary Report for Refuel Outage 14 for submittal to the NRC, The ASME XI code requires that this report be submitted within 90 days from the date of the last inservice examination, which took place on May 13, 2003.

the attached report is a tabulation of all inservice examinations and code repairdreplacements performed at Pilgrim Station from th,e end of RFOI 3 through RFO14. Exceptions to the RFOI 4 scheduled examinations as described in Entergy letter to NRC 1.2.03.45 are as follows:

o Four Recirculation system N2 inlet nozzle dissimilar metal safe end welds were not examined as planned due to automated scanner mechanical problems experienced by the inspection vendor. The four welds will be examined in accordance'with BWRVIP-75 and Supplement 10 Appendix VIIVPDI requirements during RF015, the next refueling outage at Pilgrim.

Eight RPV vertical shell welds were examined using the IHi AIRIS remote scanning tool as opposed to the six scheduled welds. The remaining 4 vertical shell welds will be examined during RF05.

0 The entire RPV shell-to-flange weld was examined during RFOl4 from the vessel interior side from two directions using the IHI AlRlS tool with Appendix VIIVPDI techniques.

I) A total of 65 ultrasonic examinations for flow-assisted corrosion effects were performed in accordance with Generic Letter 89-08. Seven planned examinations were postponed due to schedule constraints.

1.0 EXECUTIVE SlJMMARY This report documents the inservice inspection (ISI), preservice inspection (PSI) and augmented examinations performed at Pilgrim Nuclear Power Station (PNPS) for the period from the end of Refueling Outage 13 to the end of Refueling Outage 14. The examinations performed for all ASME Section XI code categories are included in the third period of the Third Ten Year Inspection Interval of the PNPS IS1 Program.

Pilgrim has inspected Class 1 category B-J and 3-F welds since 2001 (RF013) using a partial-scope Risk-Informed IS1 Program in accordance with NRC Safety Evaluation Report (TAC No, MB0841) as detailed in NRC letter to Entergy 1.1.01.046. Pilgrim also employes BWRVIP-75 scheduling guidelines for Category C, D and E austenitic stainless steel piping welds in accordance with the NRC Safety Evaluation Report (TAC No. MA5012) dated September 15, 2000 as an alternative to Generic Letter 88-01 inspection schedules.

Types of examinations Derformed:

ASME XI inservice and preservice visual, surface and volumetric examinations of piping, bolting, valves and supports.

ASME XI subsection IWE visual and uItrasonic examinations of containment per PNPS procedure QA 20.03 and the General Visual Walkdown of Primary Containment in accordance with PNPS procedure 2.1.8.7.

Augmented manual ultrasonic (UT) examinations for intergranular stress corrosion cracking (IGSCC) per Generic Letter 88-01 as modified by BWRVIP-75.

Augmented UT examinations of high energy piping for flow-assisted corrosion (FAC) per Generic Letter 89-08 and Specification M-577.

Visual examination of invessel Core Spray piping in accordance with BWRVIP guidelines, in addition to various other invessel visual examinations.

0 Augmented UT examination of one Salt Service Water piping spool in accordance with Specification M-591 (Generic Letter 89-13).

System pressure tests for IS1 Class 2 and 3 piping for the second period of the Third Interval IS1 Program.

Examination Details Piping, bolting and valve examinations were conducted in accordance with ASME Section XI 1989 edition, Code Case N-491 for supports and Entergy letter to NRC 1.2.03.45 which provided the scope of the on-line and RF0#14 outage examinations. Containment examinations were performed in accordance with ASME XI 1992 edition with 1992 Addenda subsection W E as modlfied by the Code of Federal Regulations. Scientech (NES)

Corp. was the vendor for a portion of the online scope and all non-RPV IS1 examinations performed during the refueling outage. Framatome COT. performed all nozzle and Reactor Pressure Vessel weld examinations during RFO 14. General Electric Corp. performed all invessel examinations during the outage. Entergy Quality Control personnel performed the balance of on-line examinations prior to RF0#14.

Regarding Entergy letter 1.2.03.45, Pilgrim Station 2003 On-Line and RF014 Inservice Inspection (ISI) Plan:

All planned examinations described in the IS1 plan Ietter were completed with the exception of certain IVVI examinations and four Recirculation system nozzle-to-safe end dissimilar metal welds (described below).

The examination of four N2 nozzle-to-safe end dissimilar metal welds (NZD, E, F, J nozzles) in accordance with BWRVIP-75 and Supplement 10 Appendix VWPDI requirements was deferred to RF01.5 due to automated scanner mechanical problems experienced by the IS1 inspection vendor.

In addition to the 6 RPV shell vertical welds planned for RF014 examination, two other vertical shell welds were also examined using the MI AIRIS remote scanning tool. The remaining 4 vertical shell welds will be examined during RF015.

The entire RPV shell-to-ff ange weld was examined during RF014 from the vessel interior side from two drections using the MI ACRIS tool with Appendix VIIYPDI techniques.

A total of 65 ultrasonic examinations for flow-assisted corrosion effects were performed in accordance with Generic Letter 89-08. Seven examinations were postponed due to schedule.

Inservice visual examinations of replaced CRD bolting were not performed due to a recent change in 10CFRSO that eliminates this inspection requirement for bolting that 1s not re-used.

Ultrasonic wall thickness examination was performed (on-line) on one rubber-lined carbon steel Salt Service Water (SSW) pipe spool in accordance with Specification bl-591 and Generic Letter 89-13. The planned replacement of 7 SSW pipe spools was completed as part of the SSGV Pipe Replacement Project during RFO14.

In Vessel Examinations (by General Electric)

The report of the invessel activities has been provided to the BWRVIP database maintained by EPRI. A summary of the results is provided here.

All examinations stated in Entergy letter 1.2.03.45 were completed with the following two clarifications. No indications were found.

Three jet pump beams were visually examined for mid-span cracking.

One Core Spray weld was examined to IE Bulletin 80-13 sensitivity.

Containment Examinations (IWE)

Augmented and program examinations were performed for the containment IWE examination program in accordance with PNPS procedure QA 20.03 and ASME XI 1992 edition with 1992 Addenda as modified by the Code of Federal Regulations.
