IMS-228 'A' SG Header Drain and Trap Isol is OOS. Unit I TSAC 3.6.3.C2 to verity the affected flow path is Isolated once per 31 days Unit 2 Is In MODE I at 100% power.2RC-508. Reactor Makeup Water Containment isolation Valve Is OS, Unit 2 TSAC 3.6.3A.2 to verify Isolation once per 31 days.

_.Entry.Text Unit 1-during RESP 3.1 Primary Systems Tests. Control Bank B rod F-8 dropped unexpectedly. The rod was fully withdrawn wfhen the rod unexpectedly dropped. Rod botton tight and IRPI Indicates rod on bottom. (Bank B had been fully withdrawn at 228 steps with banks C and D moving wvhen the drop occurred - approximately 116 additional steps taken after bank B reached 228 steps). (Jessessky. Tom -
