Dear Mr. Smith:

The Environmental Survey and Site Assessment Program (ESSAP) of the Oak Ridge Institute for Science and Education (ORISE) performed confirmatory survey activities at the subject facility on March 17, 2004. Survey activities included document and data reviews; alpha, beta and gamma surface scans; and, surface activity measurements.

Enclosed are five copies of the subject report which describes the survey procedures and results.
