Cook Nuclear Plant Units 1 and 2 REQUEST FOR INITIAL LICENSE EXAMINATION SUPPORT Indiana Michigan Power Company (I&M), the licensee for Donald C. Cook Nuclear Plant, is requesting Initial License Examination support by Nuclear Regulatory Commission (NRC) Region I11 personnel in August 2004. This support is needed to write and administer one senior reactor operator upgrade re-take examination.

During conversations held between Mr. Dell McNeil and Mr. Michael Bielby (NRC Region 111), and Mr. Ronald Sieber and Mr. Ivan Fleetwood (I&M), it was determined that the development of the re-take examination would require writing the NUREG-1021, Revision 8, Supplement 1, Category A, Administrative Topics, portion of the examination. It is our understanding that Region I11 staff has agreed to write the applicable section of the examination.

The examination is to be validated and administered the week of August 9,2004.

There are no new commitments identified in this submittal.

DONALD C. COOK NUCLEAR POWER PLANT, UNITS 1 AND 2 SCHEDULING OF AN INITIAL LICENSE RE-TAKE EXAMINATION

Dear Mr. Nazar:

In a letter dated July 19, 2004, (AEP:NRC:2574-62) your staff requested the NRC develop and administer a senior reactor operator upgrade re-take examination for an applicant that failed the initial operator license examination administered in March 2004, at the Donald C. Cook Nuclear Power Plant Units Iand 2. In a telephone conversation on July 20, 2004, between Mr. I. Fleetwood, Initial License Training Lead, and Mr. D. McNeil, Reactor Engineer, arrangements were finalized for the development and administration of the requested re-take examination. On August 10, 2004, the applicant will be re-examined at the Donald C. Cook Nuclear Power Plant training building. The re-take examination will cover Category A, "Administrative Topics," of the operating test.

Per your request, the NRC will prepare the re-take examination based on the guidelines in Revision 8, Supplement 1, of NUREG-1021, "Operator Licensing Examination Standards for Power Reactors." Your staff will be given the opportunity to review the proposed examination immediately prior to administration of the examination. In the unlikely event that significant changes are required for the examination to be valid due to plant administrative procedure revisions, the examination will be updated and administered on August 11, 2004.

To meet the above schedule, it was necessary for your staff to furnish the plant's operations administrative procedures during the week of July 19, 2004. Arrangements were made to send the procedures by e-mail to the NRC examiner that will develop the examination.

Appendix E of NUREG 1021 contains a number of NRC policies and guidelines that will be in effect while the re-take examination is being administered.

A signed application certifying that all training has been completed should be submitted at least 14 days before the examination date. The application should include a request for waivers for all parts of the examination the applicant passed on the first license examination. Those waivers will be granted upon receipt of the application. If you are unable to provide the final

OPERATING TEST COMMENTS FOR DC COOK RETAKE TEST (8/10/04)

There were no facility comments on the proposed retake test which consisted of five administrative job performance measures. The proposed and administered job performance measures were the same. Only one set was submitted to ADAMS.
