APPENDIX A NOTICE OF VIOLATION Louisiana Power & Light Company Docket:

50-382 Waterford Steam Electric Station, Unit 3 Operating License:

NPF-38 During an NRC inspection conducted November 16 through December 15, 1987, a violation of NRC requirements was identified.

The violation involved failure to maintain adequate design control over a safety related component.

In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1987), the violation is listed below:

Criterion III of Appendix B to 10 CFR Part 50, the licensee's approved quality assurance plan, and the approved Nuclear Operations Management Manual,Section V, Chapter 3, Revision 2, " Design Control" require, in part, that measures be established for the selection and review for suitability of application of parts that are essential to the safety-related functions of components.

Contrary to the above, partially threaded bolts were inappropriately installed in the hub of Dry Cooling Tower Fan 4B in 1983, resulting in the failure of one blade in November 1987.

Similar incorrect bolts were also installed as replacements in November 1987, when the failed blade was replaced.

This is a Severity Level IV violation. (Supplement I)(382/8725-01)

Pursuant to the provisions of 10 CFR 2.201, Louisiana Power & Light is hereby required to submit a written statement or explanation to this Office within 30 days of the date of the letter transmitting this Notice. This reply, should include for each violation:

(1) the reason for the violation if admitted, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved.

Where good cause is shown, consideration will be given to extending the response time.
