Independent Inspection Effort a.
Concrete Placement and Related Activities (Unit 1)
The inspector observed partial placement of Category I concrete pour Al-K4 at elevation 467 feet in the Unit I reactor building area.

Acceptance criteria examined by the inspector are specified in the following documents:
(1) Section 3.8 of the PSAR and TVA Topical Report 75-1A (2) TVA Specification G-2, " Plain and Reinforced Concrete" (3) Quality Control Instructions C-201 through C-214 (4) Drawing Series 4RE0417-5R, R8, Walls and Slabs (5) Drawing Series 4RE0418-5R, R3, Reinforcement (6) Specification N8C-887, " Plain and Reinforced Concrete" Forms were tight and clean and preplacement inspection was indicated by the signed pour card. Areas inspected included delivery, conveying, placement, consolidation and curing of concrete.
The following records were reviewed by the inspector for pour Al-K4:
(1) Concrete Pour Card (2) Concrete Sample Data Sheet (3) Mixing Plant Report (4) Concrete Curing Report (5) Slump Control Record Concrete operations were continuously monitored by Civil QC personnel.
Concrete delivery, conveying, placement, consolidation and curing were conducted in accordance with applicable procedures.
No items of noncompliance or deviations were identified.

Quality Assurance (QA) Audits (Units 1 and 2)
The following audits conducted by the site QA Unit were examined by the project inspector for completeness, deficiencies found, corrective action taken and implementativu in attordance with the licensee's procedures QASP 7.1 and CEP 18.01:

Work Package System (Units 1 and 2)
The project inspector reviewed the work package system as implemented on site. The following procedures are applicable:
S0P 11 Preparation, Review, Issue, Execution, and Closure of Work Packages CEP 5.04 R3 Work Packages These procedures were reviewed by the project inspector.
S0P 11 augments and clarifies the guidance given in CEP 5.04.
Areas inspected included preparation of work packages, review by applicable QC/ Project personnel, identification and documentation of QC hold points, identification of applicable drawings and procedures, control of revisions and addendums, and closure of work packages.

Open work package D440-M-2 was also reviewed. This work package covers fabrication and installation of three-quarter nch ASME Class 3 chill water piping. Welding of this piping was previously inspected and reported in IE inspection report 566-567/79-10. Documentation of all required QC hold points was on file in the Document Control Unit.
Hydrostatic testing had been completed and documented, but a noncon-formance report (NCR) was in preparation to identify and resolve the application of incorrect test pressures to portions of the chill water piping.
Preparation, review, issue, execution and final closure of work packages inspected was in accordance with applicable procedures.
No items of noncompliance or deviations were identified.

This item remains open pending a backfit review by the site QA unit of those quality related SOP's issued prior to the implementation of QASP 4.6.

(Closed) Item 566-567/79-09-02:
Stop Work Procedure. The licensee has prepared and issued CEP 1.03, "Stop Work" that provides procedures for an employee to stop safety-related work activities which would result in an adverse condition.
This procedure applies to all project personnel. QASP 7.4, "Stop Work" provides for the use of CEP 1.03 by site QA Unit personnel to stop work on safety-related activities.
No items of noncompliance or deviations were identified.
7.
IE Circulars (Units 1 and 2)
The file of IE Circulars maintained by the Document Control Unit was examined.
IE Circulars through IEC 79-20 had been received on site and reviewed by responsible project management.
If an IE Circular describes a problem that has potential applicability to the site, a determination is made by the site and/or design engineering organizations of the extent of the problem and the required action to correct the problem is initiated.
