Unresolved Items Unresolved items are matters about which more information is required in order to ascertain whether they are acceptable items, items of noncompliance, or deviations. Two unresolved items are discussed in paragraph 5.
4.
2ndependent Inspection Effort The inspector made a visual inspection of construction activities and observed QA implementation, work performance and records in the following areas:
a.
Class B, C and D storage facilities b.
Calibration of batch plant scales c.
Mixer uniformity tests on the batch plant d.
Petrographic report on concrete aggregates e.
Preliminary concrete mixture design and proportioning f.
Test data en concrete aggregates c:aent, water and admixtures g.
Calibration records on batch plant scales h.
QCIR's 4142, 4170 and 4171 Work performance, records and QA-QC controls were in accordance with licensee commitments. No deviations or items of noncompliance were identified.
5.
Containment Structural Concrete - Review of Quality Assurance Implementing Procedures (Units 1 and 2)
The inspector reviewed specifications and procedures concerning control of structural concrete. Acceptance criteria examined by the inspector appear in Sections 3.8 of the PSAR and TVA Topical Report TR 75-1A.
Controlling procedures and specifications examined by the inspector are:

TVA Specification G-2, " Plain and Reinforced Concrete" b.
Quality control instructions on inspection of concrete and concrete materials, QCI-C-20 through QCI-C-214, QCI-C-401 and QCI-C-40.

Specification N8C-887, " Plain and Reinforced Concrete" e.
SOP-8, " Calibration Procedure for Mixing Plant Scales" f.
Draft procedure titled, " Calibration. of Measuring and Testing Equipment" Examination of the above procedures and specifications disclosed the following unresolved items:
a.
QCI 208, " Compressive Strength, Slump, Air Content and Unit Weight" appears to be in conflict with paragraph 3.8.3.6.1.1 of the PS AR. QCI 208 specifies that concrete test cylinder shall be taken every 100 cubic yards for each class of concrete whose production exceeds 100 cubic yards. The PSAR specifies that at least one sample for concrete test cylinders shall be taken for each 100 cubic yards of concrete or a minimum of one sample for each class of concrete produced. QCI 20S does not appear to require test cylinders be made for each class of concrete whose placement is less than 100 cubic yards while the PSAR requires test cylinders be made for each class of concrete whose placement is less than 100 cubic yards.
The apparent conflict between QCI 208 and the PSAR on f requency of sampling for test cylinders was identified as Unresolved Item 566/78-10-01 and 567/78-10-01.
b.
Table 17-1A-4 of Topical Report 75-1A indicates the licensee will comply with ANSI N45.2.5-1974 except as specified in PSAR Section 3.8.3.6.1.
ANSI N45.2.5-1974 requires that pumped concrete must be sampled at the pump line discharge. No exceptions are made to this requirement in PSAR Section 3.8.3.6.1.
The specifications and procedures do not specify the location where pumped concrete is to be sampled.
The apparent conflict between the PSAR and specifications and procedures on the location for sampling pumped concrete was identified to the licensee as Unresolved Item 566/78-10-02 and 567/78-10-02.

Site Preparation - Observation of Work and Work Activities, Units 1 and 2 The inspector examined ongoing work and results of work for excavations of Units 1 and 2 powerhouse blocks. Acceptance criteria examined by the inspector appear in sections 2.4 and 2.5 of the PSAR and TVA Topical Report TR 75-1A.
Controlling procedures.and specifications examined by the inspector are:

N8C-882, R0, Earth and Rock Foundativns and Fills S0P No. 4, Release for Blasting After Placement of Concrete c.
d.
S0P No. 5, Control of Blasting Materials Foundations are on silty limestone of the Ft. Payne fo rma tion (Mississippian age).
Excavations in bedrock are by controlled blasting using presplitting to minimize overbreak.
Observation of results of blasting and shot number B243 in Unit Number 1 equipment sump indicated blasting is being controlled in accordance with specifications and procedures.
Dewatering is being controlled by ditches and sumps. Documentation of foundation preparation is through photographs and geologic mapping by the project geologist.
Final approval of foundations before placement of concrete is through a foundation i tspection team made up of Knoxville and site engineers and geologists.
No deviations or items of noncompliance were identified.
7.
Site Preparation - Review of Quality Records, Units 1 and 2 The inspector examined the blast release monitoring log, photographic record of excavations, compaction test curves and moisture penetration test data for borrow materials, and blast records on shot number B243.
Acce,tence criteria examined by the inspector are listed in paragraph 6.
Examination of blast records, photographic records and soils test data indicate site preparation activities are being accomplished in accordance with acceptance criteria.

Exit Interview The inspector met with the licensee representatives dencted in paragraph I at the conclusion of the inspection. The licensee was apprised of the scope of the findings of the inspection which included construction status, QA program implementation, work performance, and quality records for structural concrete, site preparation and storage facilities. The licensee acknowledged the two unresolved items listed in paragraph 5 and indicated measures would be taken to clarify the items.
The inspector stated that no deviations or items of noncompliance were identified.
