ENCLOSURE 1 NOTICE OF VIOLATION Carolina Power and Light Company Shearon Harris Unit 1 Docket No. 50-400 License No.
CPPR-158
The following violation was identified during an inspection conducted on May 20, 1985 - June 10, 1985.
The Severity Level was assigned in accordance with the NRC Enforcement Policy (10 CFR Part 2, Appendix C).
CFR 50.55(f)(1) requires CP8L to implement the quality assurance program described or referenced in the Preliminary Safety Analysis Report.
Section 1.8.5.5 of the COL guality Assurance Program requires that measures be established to ensure that inspections are conducted in accordance with documented instructions, procedures, and approved drawings.
Contrary to the above, on May 25, 1985, a site start-up engineer, while conducting a
preoperational test by procedure 1-2080-P01, directed an auxiliary operator to close the manually-operated suction and discharge valves for charging pump 1A.
The procedure did not allow or require the valves to be closed.
The subsequent test steps required running the pump.
The pump was started, with both valves closed, causing an overheated condi-tion and ultimately a pump failure.
This is a Severity Level IV violation (Supplement II).
Pursuant to 10 CFR 2.201, you are required to submit to this 'office within 30 days of the date of this Notice, a written statement or explanation in reply, including:
(1) admission or denial of the alleged violation; (2) the reasons for the violation if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved.
Security or safeguards information should be submitted as an enclosure to facilitate withholding it from public disclosure as required by 10 CFR 2.790(d)
or 10 CFR 73.21.
