Inspection Summary Inspection on March 1 through April 15, 1983 (Report No. 50-483/83-05(DPRP))
Areas Inspected:
Inspection of safety-related activities in the areas of construction, preoperational testing, and operations. Activities inspected l
included electrical and instrument cable installation and termination; l
conduit and cable tray installation and supports; piping and pipe supports; and management controls for preoperational testing. The inspection involved a total of 192 inspector-hours onsite by the Senior Resident Inspector including 9 hours1.041667e-4 days <br />0.0025 hours <br />1.488095e-5 weeks <br />3.4245e-6 months <br /> of inspection performed during non-regular hours.
Results: Three items of noncompliance were identified:
(untimely audit response, inadequate procedures, inadequate delineation of authorities and responsibilities).

Licensee Action on Previously Reported Items (Closed) Open Item (483/79-01-02):
Item was pending reviews concerning IE Circular 78-08.
The circular was closed in inspection report 82-15.

(Closed) Unresolved Item (483/80-05-01):
Item was issued to track a 10 CFR 50.55(e) report involving undersized socket welds. The 10 CFR 50.55(e) item was closed in report 82-05.

(Closed) Unresolved Item (483/80-11-07):
Item involved clarification of recording elevations on lifts during placement of backfill.
Licensee has changed form to reflect average elevation of lifts. This item is consi-dered to be resolved.
(Closed) Severity Level IV violation (483/81-14-04): Failure to inspect cable trays prior to cable installation. The trays identified have been inspected.
Procedures for installation and quality control inspection have been revised to include criteria for inspection prior to installing cable in raceways. This item is considered closed.

Inspection and Enforcement Circulars (IEC) Followup For the IE Circulars listed below, the inspector verified that the circular was received by the licensee management, that a review for applicability was performed, and that, if the circular was applicable to the facility, appropriate corrective actions were completed or had been initiated.

Plant Tours One or more plant areas were toured several times each week during the reporting period to observe general construction and testing activities, area cleanliness and storage conditions of components, piping systems and electrical / instrument cable. During these tours the inspector also interviewed craf ts persons, supervisors and quality inspectors.
No items of noncompliance or deviations were identified.
5.
Piping and Supports The inspector observed work in progress and examined completed instal-lations of pipe and pipe supports in the diesel and control building, the auxiliary b*iilding and the reactor building. The major portion 'f the inspection was performed on the 1974 and 2000 levels of the auxiliary building. Work packages and weld travelers were at the work location.
Weld material control was in accordance with approved procedures. Welders observed performing welds were qualified to the applicable weld procedure specification and code for the welding being performed.
Inspection hold or witness points were being observed in accordance with requirements.
No items of noncompliance or deviations were identified in this area.
6.
Electrical The inspector examined electrical cable installation in the reactor building, auxiliary building and the control building.
Generally, the cable was being protected from physical damage before and after installa-tion and, in those instances where the cable was inadvertently damaged, the cable was either replaced or repaired according to approved repair procedures.
In those areas inspected, the latest approved drawings and work procedures were being utilized for cable installation and terminations. The inspector found no deficiencies in cable location, routing or electrical separation between redundant divisions in the reactor or auxiliary buildings.
Cable tray and conduit loadings were within specified limits or overfills had been previously identified by quality inspectors and reported to the designer.
The inspector observed cable termination activities on penetrations and in panels in the electrical penetration rooms. The latest instructions and drawings were on hand.
Personnel performing the terminations were certified according to approved certification procedures.
No items of noncompliance or deviations were identified in this area.
7.
Instrumentation The inspector examined installation of instrument cables in the reactor and auxiliary buildings. The cables appeared to be installed according to the

latest approved procedures and drawings. Cables and wireways were identified according to requirements.
Locations routing protection and separation in the reactor and auxiliary buildings were in accordance with specifications and standards.
No items of noncompliance or deviations were identified in this area.
8.
Preoperational Testing The inspector examined portions of the preoperational testing / start-up program including activities related to management control, quality assur-ance and the inspection or surveillance of safetyrelated test activities.
a.
During the inspector's examination of the Test Program Surveillance
,
Group activities, the following was noted:
(1) Procedures for performing surveillance activities had been written and approved by a former group leader. There was no indication that these procedures had been reviewed or approved by responsible management personnel.
(2) There appeared to be no procedural requirements for witnessing tests nor for establishing hold or witness points during the performance of tests.
(3) Schedules for routine surveillances had not been developed since November 1982, and no reason for discontinuing the surveillance schedule could be determined.
The inspector informed the licensee that the performance of safety-related activities without adequate documented, approved procedures was a violation of 10 CFR 50, Appendix B, Criterion V (483/83-05-01).
b.
The inspector observed that the program did not clearly delineate responsibilities and duties of the various groups and persons within the Start-up Organization. No clear interfaces between elements had been established.
Stop-work authority had not been established within the organization except that the person performing a test was

permitted to terminate the test.
The inspector informed the licensee that the failure to clearly delineate the duties and responsibilities of persons and organizations performing safety-related functions was a violation of 10 CFR 50, Appendix B, Criterioan I, (483/83-05-02).
c.
The inspector exan.ined four reports of audits performed by licensee Quality Assurance Organization and twenty-three reports of surveil-lances performed by the Test Program Surveillance Group. Of those surveillance reports examined by the inspector, responses were overdue on five reports. One was routed to a Start-up engineer whose response was that he had no authority to initiate the recommended corrective action. The item was closed on this response with no other apparent followup.

The audit of June 15-18 1982 had six findings. Three of t 2 findings involved temporary alteration control.
Corrective action and close out of these three items had not been accomplished at the time of the
February 22-28, 1983, audit or as of the date of this report.

Exit Interview LThe inspector met with licensee representatives (denoted under Persons Contacted) at intervals during the inspection period. The inspector summarized the scope and findings of the inspection. The licensee acknow-ledged the fin?:ngs as reported herein.
