M Inspection Summary Inspection on February 1-19, 1982 (Reports No. 50-454/82-04(DPRP);

50-455/82-03(DPRP))

Areas Inspected: Routine safety inspection to review pre-operational test activities, maintenance procedares and other plant activities. The inspec-tion involved a total of 32 inspector-hours on site by one NRC inspector including four inspector-hours onsite during off shifts.

Results: Of the three areas inspected no apparent items of noncompliance or deviations were identified.

Pre-operational Test Program a.

The inspector observed portions of the test performance of Diesel Fuel Oil-Emergency Diesels "D0" of Byron Preoperational Test-Procedure 2.24.10.

The inspector observed briefings, conduct of the test, observation of precautions, calibration and placement of test instrumentation, documentation of results and unusual event /

deficiencies.

The inspector observed during reperformance of a test section that alarm 16COS failed to alarm as required in the control room.

Subsequent information provided by the test engineer indicate the alarm circuitry had been disconnected by construction personnel using approved construction procedures, that the alarm had been previously tested satisfactorily, that the circuitry belonged to the "DG" system and that the test engineer had not been informed that circuitry affecting the test was being worked on by con-struction personnel. Further investigation by the inspector revealed that the operations department does not maintain control of work in an electrical panel unless the construction department has formally turned over control of the panel to the operations department even though one or more cables in the panel are under operational control and have been tested. The inspector expressed a concern to the licensee that a potential exists for inadvertent

lifting of tested safety related equipment electrical wiring, that controls do not exist for identification of inadvertently lifted leads, necessary reconnection verification, and subsequent retest identification and accomplishment if required. The licensee com-mitted to prepare, approve and issue a procedure to require, during inspection of safety related cable for separation purposes, wire-check verification for proper installation and include re-inspection requirements for any work subsequent to the initial verification.

This matter is considered open pending issuance of the procedure by the licensee, review of the procedure by inspector and verifi-cation of proper implementation (454/82-04-01; 455/82-03-01).

The inspector observed portions of the test performance of Residual Heat Removal "RHR" of Byron Preoperational Test Procedure 2.67.10.

The inspector observed verification of prerequisites, briefings, conduct of the test, observation of precautions, calibration and placement of test instrumentation, documentation of results and unusual event /defiencies. No apparent items of noncompliance were identified.

The inspector observed portions of the test performance of Diesel Generator-Emergency Diesels "D.G." of Byron Pre-operational Test Procedure 2.22.10.

The inspector observed verification of pre-requisites, briefings, conduct of the test, observation of precautins, calibration and placement of test instrumentation, documentation of results and unusual event /defiencies. No apparent items of noncompliance were identified.

Maintenance Procedures Review The inspector reviewed selected plant instrumentation procedures to confirm that the plant maintenance procedures are prepared to adequately control maintenance of safety related systems within applicable regula-tory requirements. The licensee committed to issue a change to the Jumper / Lifted-Lead Log procedure to include specific requirements for two party verification subsequent to component restoration. This matter is considered open pending verification of the corrective action and proper implementation of the corrective action (454/82-04-02; 455/82-03-02).

Exit Interview The inspector met with licensee representatives (denoted under Persons Contacted) at the conclusion of the inspection. The inspector summarized the scope of the findings of the inspection. The Licensee acknowledged the findings reported herein.
