Calibration records not reaiily traceable to National Bureau of Standards or equivalent - Newport News Industrial Corporation, Units 1 and 2.
The licensee conducted an audit of Newport News Industrial Corporation calibration program on or about October 5, 1978; as a result, several significant conditions were documented in CAR 0450.

CE) Action Relative to RIII Immediate Action Letter of February 8, 1978, Units 1 and 2 Relativt to item 8 of the RIII Immediate Action Letter of February 8, 1978:
a.
The inspector reviewed the CEI Project Administration Manual Procedures 1002, 1301, 1003 and n201.
b.
As a result of the procedure review, the inspector noted that commitments to Regulatory Guides 1.37, 1.38, 1.58 and 1.74 are delineated in CEI Project Administratior Procedures 1002, 1301, 1003 and 0201, respectively.
Item 8 of the RIII Immediate Action Letter of Februarv 8, 197S, remains open pending the inspecter's review of CEI Project Administration Procedures 1001, 0202, 0901, 0301, 1701, 1004, 1005 and 040i; NOTE:
These procedures have not yet been developed bv CEI.
Further evaluation of site contractor's quality assurance contract specifications will be made, on a selective basis, by the RIII inspector af ter all of the af orementioned proj ect administrative procedures have been developed.
No items of noncompliance were identified.

Quality Assurance Program Review - Instrumentation, Units 1 and 2 a.
The inspector reviewed selective sections of the Johnson Controls Inc., QA program.
Johnson Controls Inc., is an instrumentation contractor with installation / inspection responsibilities for safety related instrumentation at the Perry Nuclear Plant.
Those sections which were selected and reviewed are as follows:
(1) Organization Structure and nA Personnel (reviewed Johnson's QA manual, Section 3 and Procedure QAS-201, Revision 0).

(2) Procurement Document Control (reviewed Johnson's QA manual, Sections 5 and 9 and Procedures QAS-501, Revision 1 and QAS-702, Revision 1).
(3) Work and Quality Inspection Procedure requirements (reviewed Johnson's QA manual Section 6 and Procedure QAS-402, Revision 1).
(4) Material Control (reviewed Johnson's QA manual Sections 9 and 14 and Procedures QAS-1401, Revision 0 and QAS-1601, Revision 0).
(5) Document Control (reviewed Johnson's OA manual Sections 2 and 7 and Procedure QAS-402, Revision 1).
(6)

The inspector compared the above procedures with the Ouality Assurance requirements which are specified in the CEI/F-C contract specification, SP-709-4549-00, dated August 12, 1974; 10 CFR 50, Appendix B; and ANSI N45. 2-1971.
(1) The inspector had concerns about training and quali-fication requirements for inspection personnel.
(2) The inspector was also concerned that Johnson's Procedures QAS-1802 and QAS-1301 have not yet been submitted to CEI for approval.

With respect to items (1) and (2) above, the inspector was informed by Gilbert QA personnel, that Johnson Controls Inc. has been instructed to revise the applicable procedures to ass.ure that tim requirements of SP-709-4549-00, Paragraph 1:06.2 (in reference to the qualification and training rtquirements of ANSI N45.2.6) are complied with and that Johnson Controls Inc. will be required to have Procedures QAS-1802 and QAS-1301 submitted for review and approval in a timely manner to assure that the applicable work does not proceed until these two procedures have been approved by CEI.
At this time, the inspector has no further cuestions about these matters.

Fire Prevention / Protection a.
The inspector ascertained that there were procedures and methods established relative to a fire emergency during construction activities which included:
(1) NFPA standards for construction.
(2)
Fire brigades.
(3) General housekeeping, including flammable liquic and materials.
(4) Alarm system.
(5) Watch service (Security Force).
b.
P4 -indic insnections are performed by the insurance inspector.
c.
There is an organized fire brigade made up of designated personnel, who received training in the use, cart and locations of hydrants, noses and fire extinguishers.
The training included the proper techniques for fighting and isolating the various types of fires.
Training records are maintained by the Security Department.
d.
The inspector observed several fire stations, including water hydrants and dry chemical extinguishers.
General observations are as follows:
(1) Water hydrants were pressurized. Tocls were located inside cabinets at the stations.
There was no evi-dence of deterioration of hoses.

(2) The inspection log indicated that the dry chemical fire extinguishers were being inspected monthly.
(3)
Several welding and flame cutting operations were observed throughout the buildings.
The inspector reviewed three subcontractor's welding procedures and the Plant Protection Manual for the site.
Only one of the subcontrarcor's welding procedures referenced a fire watth/ safety pre-caution requirement.
The licensee's Security Department is in the process of incorporating fire watch requirements into the Plant Protection Manual, which is distributed to all site personnel and subcontractors.
The inspector has no further questions on this matter at this time.
No items of noncompliance were identified.

Quality Assurance Implementing Procedures Review - Electrical, Units 1 and 2 a.
The inspector reviewed selective Ernst-Comstock OA/nC procedures. Those procedures which were selected and reviewed are as follows:

Cnntrol and Identification of Equipment and Components, E-C Procedure 4.6.1, dated October 10, 1978.
(2)
Field Engineering Changes. E-C Procedure 4.2.2, dated June 1, 1978.
(3) Cable Tray and Conduit Installation, E-C Procedure 4.3.1. dated October 10, 1978.
(4) Cable Tray and Conduit Identification, E-C Procedure 4.3.2, dated October 10, 1978.
(5) Work Instructions for Field Storage and Paintenance of Class 1E Motors, E-C Procedure 4.3.15, dated December 1, 1978.
(6) Welding Procedure for Galvanized Sheet and Tubing, E-C Procedure 4.7.7, dated August 1, 1978.

3.
Review of Quality Records - Cable Trays The RIII inspector reviewed Ernst-Comstock's receiving inspection documents dated Neverber 28, 1978, for Class IE cable trays which included the following:
Statement of Conformance (6); mill test certs; and seismic analysis report which was reviewed and accepted by Gilbert Associates, Inc.
No items of noncompliance were identified.
Plant Tour The RIII inspectors observed a nonseismic drain line routed over a Class 1E (seismic 1 counted) cable tray in the Control Complex Building.
The inspectors were informed that this matter had been identified to the A/E's field engineer and that corrective actions would be taken.
The inspectors expressed concern relative to similar installations involving other nonseismic piping in close proximity to safety related components which could adversely affect these components in the event of an earthquake.
The licensee informed the inspectors, prior to the exit interview, that Site Engineering will establish a log book to track those cases uhere seismic 1 systems / components are mounted under nonseismic 1 systems / components in order to assure that appro-priate actions are taken to correct the condition prior to plant completion.
The inspectors have no further questions on this matter at this time.
No items of noncompliance were identified.
Exit Interview The inspectors met with site staff representatives (denoted under Persons Contacted) at the conclusion of the inspection on December 8, 1978.

The inspectors summarized the purpose and findings of the inspection.
During the exit interview, the inspectors were informed that reevaluations of site contractor's QA programs are being made.
The reevaluations, which will be completed by December 30, 1978, are being conducted to:
followup on original program review audits which were the results of the RllI Immediate Action Letter of February 8, 1978 and to evaluate the initial responses received in order to verify implementation of corrective action responses to the original program review audits.
The licensee acknowledged the findings reported herein.
