II. Allegation as Understood by NRC

On October 4,1978, the NRC received a telephone call alleging that two girders, approximately 137' in length, used in the Limerick Generating i

Station reactor building crane had poor quality welds.

He described weld defects such as undercut, porosity, and over-run.

He was concerned that the girders would not receive adequate repairs.

- The' review verified that the. reactor building 125 ton crane is safety related, the girders;in question are the 129' girders designated G1 and G2, and' that-welding is required to. meet the American Welding Society.

'

i A visual inspection of the G1 and G2 girders disclosed that grinding I

operations were in progress to blend out and/or remove weld surface j

i imperfections.. Discussions of the grinding operation with a field

.welding' engineer established that five Nonconformance Reports had been issued describing the' weld imperfections and other nonconforming conditions.

The visual inspection.by the NRC inspector verified that undercut, porosity, and overlap conditions did exist in the welds on the girders G-1 and G-2.

There was evidence that many areas of the girders had

already been ground.

The' NRC inspector made selected weld size measurements and verified that the welds appear to satisfy drawing requirements in

,-

those areas examined.

Examination of the tie plate to girder welds, part No.18 on p&H Drawing No. 28A13615, Revision C, disclosed what appears to be a fillet weld between the parts. The drawing specifies a double-bevel-groove weld with a 45 degree weld preparation.

The girders have received final vendor inspections and Statements of Conformance have been issued on

June 27 and 30 1978.

The use of weld joint configurations other than those specified on the design drawings is an item of noncompliance (352/78-08-01).

The inspector reviewed the Nonconformance. Report (NCR) Nos 3206, 3287,

3297, 3308, and 3323 and. verified that the welding imperfections on the

.

girders had been identified by the licensee's quality control program on August 18 through September 19, 1978. The NCR's identified approximately

84 areas of welding defects such as; undercut, lack of fusion, and porosity.

I The undercut ranged from approximately.090" deep maximum to an average i

t value of.050, in depth. The disposition of the NCR's directs.that the defects be removed by grinding, blending, and rewelding as necessary.

The rewelding will be required for those. areas ground below' minimum

. IV. Conclusions The allegation that the 129' girders for the Limerick generating station reactor building crane.have unacceptable welding defects was substantiated.

,

However, the licensee's quality control program. identified these defects on August 18 through September 19, 1978, and recorded them for engineering disposition in accordance with his qualit'y assurance program.

The allegation that the girders will not receive adequate repairs was not substantiated in that the repairs are currently in progress and are being controlled in accordance with the licensee's engineering and quality assurance program. The adequacy of the repairs will be evaluated by the NRC upoh their completion during a routine inspection.

V. Exit Interview The inspector met with the licensee's representatives (denoted under

" persons Contacted") at the conclusion of the investigation on October 5,-1978. He summarized the purpose of the investigation and the findings.

He requested that the NRC Region I office be notified upon the completion of the welding repairs on the girders G1 & 2 in order that a final inspection might be performed.

The licensee's representative acknowledged the inspector's findings and request.
