Areas inspected: A routine unannounced inspection by a region based inspector of the licensee's Inservice Inspection (ISI) activities, inspection Procedures 3070?, 73052, 73753 and 73755 were used as guidance for the inspection.

General Conclusions and Specific Findings

Since the 1989 NRC inspection of the licensee 151 activities, the licensee has taken corrective actions, issued new procedures and implemented changes in various 151 areas that have, improved its ISI program.

Inservice Inspection - Observation of Work and Work Activities (73753)
The licensee was conducting the fourth refueling outage.
This outage was the last outage of the first period of the second ten year ISI interval, that started May 20, 1986.

The licensee's contractor for NDE this outage, was Volt Technical Services.
The inspector reviewed the qualifications and certification records for the ISI examiners and the e available visual (VT), ultrasonic (UT) quipment certifications.
and liquid examinationsonpressurizerpipingwereobserved. penetrant (PT)During this rev inspector identified a concern with the blank " Ultrasonic Calibration Data Sheet" " Manual Ultrasonic Piping Examination Data Sheet", " Wall Thickness Profile Sheet", " Manual Ultrasonic Vessel Examination Data Sheet" and " Beam Spread and Resolution Data Sheet" provided to the NDE
examiners for recording examination results.

number stamped on them, sometimes in a specific location such as a block l

The concern with the sheets is that they do not contain the (procedure number

Without this information or identification of a specific form / revision number it is difficult for an NDEexaminertoensureheisusingthelatestapplicable calibration /examinationsheet.
Tne NRC has identified examples where an out of date calibration / examination sheet was used at another facility

and all the required ISI data was not recorded.
The licensee stated this was the first year they had used these new procedures and calibration / data sheets, and they had not identified that the lack of this information could cause a problem.
The licensee stated they would review these sheets and revise their method of issuing new sheets to NDE examiners.
They may duplicate the applicable procedure sheets, as do other licensees, and add their unique stamped numbers, or they may use an alternative method which clearly identifies the correct procedures and revision number.
No violations or deviations were identified in the areas reviewed.

Inservice Inspection - Data Review and Evaluation (73755)
The inspector reviewed avai sble NDE ISI calibration sheets,/ data sheetdata sheet and NCR s generated this outage.
Except for the calibration concern identified in paragraph 3 of t11s report, no other problems were identified.
No violations or deviations were identified in the areas reviewed.
5.
Exit Meeting (30703)
The inspector met with the licensee management representatives denoted in paragraph 1 on May 3, 1990.
The scope of the inspection and the
inspector's findings up to the time of the meeting were discussed.
At this meeting the inspector identifird that he had obtained some information that would be reviewed later in the region, with the findings documented in this report.
The available information was reviewed and the findings included in paragraph 3 and 4 of this report.
