Inspection on October 18 - 21,1982 (Report No. 50-322/82-28)

Areas Inspected: Routine, inannounced inspection by one region-based inspector of the licensee's work and QC activities pertaining to corrective action for significant deficiencies and other outstanding items.

The inspection involved 28 onsite hours.

Results: No items of noncompliance were identified.

Licensee Action on Previous Inspection Findings (Closed) Unresolved Item (80-05-05): Construction of seismic Category I block walls.

Resolution of this item is included in the licensee's action in response to Bulletin 80-11 and includes control of attachments to block walls following their inspection for preparation of as-built drawings.

This item is resolved.

(Closed) Unresolved Item (80-12-01): Possibility of voids in biological shield wall concrete. The inspector examined E&DCR P-3899 and Revisions A, B, and C.

This E&DCR provided a drawing showing location of areas to be grouted and detailed grouting instructions.

Review of grouting reports showed that all but five of some 70 locations had been grouted and QC inspected. The remaining five locations are identified and awaiting

completion.

Field QC was reviewing documentation prior to close-out of this item.

The inspector had no further questions concerning this item.

Licensee Actior 'n Response to Bulletins and Circulars Circular 81-05 - (Closed) Self-Aligning Rod End Bushings for Pipe Supports.

The inspector discussed this problem with FQC personnel and reviewed correspondence between the vendor and S&W and a Bergen-Paterson sketch of Part 2003.

These show that displacement of the bushing is limited by mitered washers on the bearing pin and also that the bushings had been staked.

Bulletin 80-11 - (0 pen) Masonry Wall Design The licensee has prepared as-built drawings of the block walls showing all attachments for evaluation by engineering. A number of E&DCR's have been prepared showing the additional structural supports to be provided for a number of the walls.

Information was not available on site to determine that re-evaluation of all block walls had been completed.

Installation of additional supports was in progress.

This item remains open, pending:

(a) Completion of added supports installation and inspection of selected walls by an NRC inspector.

(b) Review of records by a NRC inspector to determine that re-evaluation of all walls has been completed, and additional supports provided where necessary.

(c) Review of provisions for control of additional attachments to block walls since completion of the as-built drawings.

Review of Non-routine Events Reported by the Licensee (0 pen) Significant Deficiency (80-00-04); Deficiencies in Internal Wiring of Square D Motor Control Centers.

Deficiencies identified and corrective actions are provided by N&D 39E8.

Since this equipment has been taken over by test and start-up, the work will be performed under Deficiency Correction Orders (DCOs).

Four DCO's have been written No. 16458, 17121, 17122, and 17221.

The inspector examined DC0 No. 16458 and QC inspection report, which shows it to have been completed.

This item remains open, pending completion of the work under DC0 No.

17121, 17122, and 1722.

(Closed) Significant Deficiency (81-00-01); Deficiency in turbogenerator thrust-bearing lubrication system for diesel generators.

The inspector examined E&DCR's F36401 and F34540, including revisions A through G, as well as Repair / Rework Requests and Final Installation Acceptance reports which showed that work required by the E&DCR's had been completed and accepted by field QC, (Closed) Significant Deficiency (81-00-03) Deficient Valve Yokts supplied by Malcolm Foundry for Anchor Darling Valves.

The inspector examined LILCo Deficiency Report (LDR) No. 0499 and Repair /

Rework Requests (R/RR) and 0QA verification reports which showdd that the necessary rework had been performed for all valves and accepted by 0QA.

(Closed) Significant Deficiency (81-00-04); Insulation Indentations on Rockbestos Cable due to Nylon Wrap.

Documentation on site shows that all Category IE cable showing significant indentation has been replaced.

Qualification test performed on cable have the greatest indentations been completed and confirm the licensee's previous conclusion that cable showing mino' indentations are acceptable for the 40 year life of the plant.

(Closed) Significant Deficiency (81-00-05); Loosening of Terminal Strips on RTD Connector Heads.

This deficiency affected 26 units on various systems. All of the c.onnector heads on these units have been replaced, as shown by N&D 3853 and the attached QC inspection reports.

(0 pen) Significant Deficiency (82-00-01); Physical Damage to Check Valve on RCIC Turbine Exhaust Line.

The licensee has replaced the two damaged check valves. However, this item remains open, pending an in-service leak inspection, as identified in R/RR E51-74 and revision of the station procedures following review of

the operational recommendations of GE Application Information Document (AID)-56.

This problem is also discussed in IE Information Notice No.

82-26.

(Closed) Significant Deficiency (82-00-04); Defect in GE HMA Relays.

The reported deficiency was a potential short circuit due to excessive length of uninsulated, flexible leads connected to the moveable contacts of some of the relays. This deficiency has been corrected by installing heat shrinkable tubing to cover the terminals and a portion of the lead in accordance with GE Field Disposition Instruction (FDI) TFHK. This work has been complete, except for two relays which have been identified and will be corrected as they become available to be worked on.

Part 21 Reports (Closed) Part 21 Report (81-88-04); Insulation Indentations on Rockbestos Cable. This is the same item described above as a significant deficiency (81-00-04).

Exit Interview The inspector met with licensee.and contractor personnel (denoted in Details, paragraph 1) at the conclusion of the inspection on October 21, 1982.

In addition, the NRC Senior Resident Inspector, Mr. J. C. Higgins, attended the meeting. The inspector summarized the scope and findings of the inspection.
