Containnent Tendon Grouting The inspector stated that in his review of contain=ent tendon stres-sing and grouting, he had fcund that in sene instances water was sepa::ating frc: the grout at the top of the tendons, resulting in inco plete g:cuting of the tops cf the stressed tendons.

This condition was under investigation during this inspection.

Condition of the Dike Riprao and Filter Layer The inspector stated that during the inspection he had participated in a review of the condition of the dike, riprap and underlying filter layer, conducted by the follewing NRC, Division of Reactor Licensing (DRL) personnel:

Batterv Cells in Contact with Uninsulated Battery Racks In a previous inspection it was f ound that station battery cells were in direct contact with uninsulated battery racks.

The licensee nas corrected this condition, per Deviation Report No. 0385, by providing plastic insulating strips in accordance with the insta.'.lation drawings.

The inspector exa=ined the battery racks and found that each glass battery case was electrically insulated frcm the battery racks.

This resolves the ite: identified in IE inspection Report No. 50-320/74-09, Details, Paragraph 2.

Containment Tenden Grouting The inspector exacinad the progress cade in the six weeks since the start of the contain=ent stressing program.

A total of eight vertical tendons had been stressed and grooted.

Examination of the grouted tendons revealed that a void had developed at the top of each vertical tendon.

The voids are several inches in diaceter and approximately 12 inches deep. They are located at the centers of the tendons and apparently result from bleeding of water which then rises to the top of tendon.

The prestressing vendor, Stressteel Company, together with its consultants, were investigating the grouting operation during the inspection. This item is unresolved.

4.

Condition of the Dike Riprap and Filter Layer The inspector participated in a review of the condition of the dike riprap and filter layer by NRC Division of Reactor Licensing personnel. The DRL hydrologists had observed apparent erosion of the dike adjacent to the river water intake structures, and has requested the licensee to provide verification that the riprap and filter layer dimensions and materials conform to the design.

Switcheear Storage Protection In a previous inspection it was found that installed switchgear was not being protected from cement dust, and environ = ental conditions were nec defined.

The licensee has revised the Electrical Control Procedure for Handling and Installing 4 KV and 480 V Switchgear, Motor Control Centers, DC Distribution Pancis and Bus Duet Procedure ECP 5-2 to specify environ-mantal storage require =ents.

The inspectoe examined selected switchgear

cabinets and found that the equipment had been cleaned, provided with plastic coverings, and heaters were activated.

This resolves the item identified in IE Inspection Report No. 50-370/74-09, Details, Paragraph 13.
