During a phone conversation on another matter, on September 11,

1972, the inspector asked the applicant's site QA Engineer if any items of quality significance had occurred at the site re-cently.

The QA Engineer mentioned possible internal da= age to the Unit 2 reactor pressure vessel, and stated that the incident was currently being investigated by Babcock & Wilcox.

B.

Description of Particular Event or Circu= stance:

A two-piece shipping cover on the Unic 2 reactor pressure vessel was being disasse= bled on 9/1/72.

A steel inner tie plate, weighing 400 pounds and measuring 1" x 12" x 15', was inadvertently unbolted, and the plate fell into the vessel.

One end of the plate lodged against an incore nozzle.

The plate end was dented indicating that it had forcibly struck some internal surface of the vessel.

Vessel da= age has not been determined due to the presence of about 10

i inches of water of unknown origin inside the vessel.

It was stated that the vessel should have been dry at that time.

The water

,

analysis (for chloride, etc) is underway, but results are not

availabic at this time.

The vessel had been fabricated and shipped from Mt. Ve rnon, Indiana.

The details of the environmental protection during shipment were not available.

The route of vessel shipment was via the Mississippi River, across the Gulf, and up the East Coast to Havre de Grace, Maryland, and thence overland to Three Mile Island.

Action by Licensee:

The applicant's project manager stated that notification to the AEC will be made in accordance with 10 CFR 50.55(e), as soon as sufficient information is known. The Babcock 6 Wilcox investi-gation vill be followed by a separate study by the applicant, including the proposed repair procedure.
