The shipment was inspected for compliance with NRC and 00T re9ulations.

Shipment listed as Item 2.0 of the enclosed report is applicable to this license. This inspection required one hour onsite by ore inspector.

Results: No items of noncompliance were identified relative to NRC and ITOT re9ulations.

Southwest Nuclear Co., Pleasanton, CA (Agreement State Licensee) Shipment Inspected: 12/10/79 '. Ca rrier: Home Transportation Trailer: 743006 Exclusive use LSA shipment-from broker cons,isting of 168 55 gallon drums of animal carcasses, scintillation vials, and dry waste. Shipping papers contained all required information.

Vehicle correctly placarded.

Packages marked " Radioactive LSA" as required. Radiation measurements at surface of vehicle and at contact with several. packages were all less than 1 mR/hr.

No removable contamination detected.

State of Nevada represent-ative made similar findings.

-International Chemical and Nuclear Corp. (ICN), Pleasanton, CA-(Agreement state licensee) Shipment inspected: 12/12/79

Carrier: Contractors Cargo Company Trailer: 58-437 Exclusive use, LSA shipment from broker consisting of 120 55-gallon drums of carcasses, scintillation vials, and solid waste, plus 196 5 gallon pails of_ depleted uranium. Shipping , papers contained all required information. Vehicle correctly placarded. Radiation measurements of vehicle and individual packages were within 49 CFR limits. No removable contamination detected onpackages. One 55 gallon drum contained no " Radioactive LSA" marking as required by 49 CFR 173.392(c)(8). No other items of noncompliance were dete:ted. No state representative was pre-sent during shipment unloading.

When the drums were laid on their sides in the burial trench, leakage of.an organic liquid was detected from many of the packages in quantities from about a pint to half a gallon.

Burial site personnel identified 28 leaking drums from nine hospitals and universities licensed by the State of California.

. . a Because burial of packages containing free standing' liquid is . not permitted by the burial site license, site management noti-fied the Nevada Department of Human Resources and the shipper.

State officials, after first considering an order to the shipper to remove the entire shipment from the state,' decided that all the leaking packages _except two should be overpacked and buried by the shipper. Arrangements were apparently being made by the State to transport two of the leaking packages to an unt.nown location in California for " thorough examination to determine the cause_of the inappropriate packaging." As a result of the incident, the shipper and nine California-licensed waste gene-rators were reportedly banned from the use of all waste burial facilities until the state provides assurance that packaging standards have been upgraded.

General Electric Company, Vallecitos Nuclear Center, Pleasanton, CA (Agreement State licensee) Shipment Inspected: 12/13/79 Carrier: Tri State Motor Transit Company Trailer: 228127 Cask (USA /9044/B) containing 1512 curies of cobalt-60 waste.

Shipping papers contained required information.

Package condi-tion was as described by NRC certificate USA /9044/B. Radiation levels at cask surface and edge of trailer were within DOT limits. Smear surveys disclosed no contamination. Vehicle placarding and package labeling were correct. Using a shielded crane, the sealed inner container, which had a radiation level , of about 1200 R/hr at contact, was removed from the cask, de-posited in the trench, and covered with soil without incident.

No state representative was present at the burial site.

Rancho Seco Generating Station, Herald, CA (NRC licensee) Shipment Inspected: 12/13/79 Carrier: Tri State Motor Transit Company Trailer: 544120 Exclusive use, LSA shipment consisting of 132 drums of dry, compactible waste and two wood boxes of dry waste. Vehicle was correctly. placarded. Radiation levels of vehicle and of packages were within limits. No detectable contamination was found on packages. All packages were correctly marked and closures were acceptable.

State representative was not present.

No items of noncompliance were identified.

Vehicle was correctly placarded.

Radiation levels of vehicle and packages were less than 1 mR/hr. No contamination was found on packages. All package closures appeared tight.

Package narkings were correct. No state representative was present at the site.
