On June 11, 1986, the NRC administered an examination to an employee of your company who had applied for a license to operate your Indian Point Unit 3 i
Facility. At the conclusion of the examination, preliminary findings were discussed with a member of your staf f identified in the enclosed report.

Weaknesses noted during the simulator examination:
DifficultieswereobservebinfollowingtheEmergencyOperatingProcedures (EOP). Steps were performed out of sequence and transitions were not made i
properly from one procedure to another. As a result, in one scenario the
'
severity of the transient was increased.
Weaknesses were also observed in providing clear and concise directions to operators. These problems indicate that more emphasis should be placed on use of the E0P's and on supervisory skills.

Problems experienced with the Indian Point 2 simulator:
Problems were experienced with the Indian Point 2 simulator, which was used for the simulator examination. The simulator was unable to operate longer than five minutes at 100% power steady state conditions without tripping on a spurious high flux signal. The simulator was unable to simulate dual failures in single-systems.

For example, the simulator,cculd not simulate an ejected rod and sub-sequent failure of rods to fasert;on the resulting reactor trip. Also the simulator was unable to prope'rly simulate a steam generator which was both ruptured and faulted.

These deficiencies are in addition to specific simulator problems noted in Examination Reports Nos. 50-247/85-16(0L) and 50-247/85-28(0L).
The candidate was not evaluated with respect to the simulator deficien-cies.
