A discussion of the steam generator tube failure was included in Ft. Calhoun NRC inspection reports numbered 84/13 and 84/14, docket number 50-285. A
' Safety Evaluation Report (SER) was prepared by the Region IV staff as a result of the tube failure. The SER, dated June 22, 1984, was entitled " Restart of the Ft. Calhoun Station Unit 1."
The additional information you requested about the incident should be contained in two letters to the 'NRC from the Omaha Public Power District.
- The first letter, with attachments, dated May 31, 1984, was addressed to J. T. Collins, Regional Administrator, by W. C. Jones. The second letter, with attachments, dated September 20, 1984, was also addressed to Mr. Collins and signed by R. L. Andrews.
