' The purpose of this inspection was to review the support structure evaluations performed for the Safety Injection and Refueling Water (SIRW) Tank as documented in LER 83-06 dated February 17, 1983 and to review and evaluate the engineering analysis and proposed method of corrective action.

. Report RS-003-03 This review compared the actual slab shear stresses to the allowable

values as specified in ACI Standard 318-63, " Building Code Requirements for Reinforced Concrete", dated June 1963 and to the requirements of the FSAR.
'An inspection conducted by the Region III specialist inspector and the resident inspector of the affected area revealed the conditions as identified and reported to Region III did in fact exist. There were a series of 3' x 3' square openings that extended through the main support beams. On the East side of the Auxiliary Building running in

line of the'SIRW Tank and penetrating the main support' beams'of the cells immediately adjacent to the center line of the tank, one on the Scuth side and one on the North side,-are.three (3) openings. These openings-display cracks. running diagonally upward on an approximately 45 angle. All three'(3) openings display cracks at the corners that are approximately 1/16" wide and follow the joint between.the top of:the opening and the West side. The cracks at the same location on the East.
side of'the openings are not as severe.
A further review of the documents listed above is being performed. This is considered to be an open item and will be addressed in more depth at a future inspection.

Procedure Review The Architech/ Engineer is in the process of writing specific procedures for the repair and inspection of the openings identified above. These will be within the frame work of the existing procedures but will be directed to the specific method of repair for these openings.
It is expected that sixty (60) days vill be needed to develop and complete these procedures. These will be reviewed in depth at that time.
This item is considered an open item and will be reviewed further during a future inspection.

Exit Interview No formal exit interview was held. However, cognizant licensee and Architech/ Engineer personnel were present at a meeting on March 29, 1983, held at the licensee's corporate facility for the purpose discussion and review of problems and the methods considered to remedy the situation.
