2.
Plant Status The plant was in a shut down mode for the replacement of welds and piping in the feedwater system of the four steam generators.
3.
Scope This inspection was made to examine the quality related activities associated with the replacement and repairs of the steam generator feedwater lines.
This work was being done as a result of the testing performed to comply with Bulletin No. 79-13.
4.
Radiography Inspection An inspection was made of randomly selected Radiographic film (approximately 30%) to assess the quality of new replacement welds made on the feedwater lines between the nozzles and the vapor containment.
As part of the random selection, the inspector reviewed radiographs of welds which had more than one repair cycle.
The radiographs were reviewed using a high intensity viewer to ascertain the following conditions for compliance with ASME Section V,1974 and USAS B31.1.
a.
Radiographic Technioues The films were reviewed for film density, film selection
"M" film, penetrameter selection and placen.ent geometric unsharpness, weld coverage and source positioning, source to film distance (SFD), weld location and identification and detectability of tne required 4T hole (s) in the penetrameter(s).
b.
Weld Quality The inspector reviewed selected radiographs (see below) and found that the weld quality was acceptable, as judged against a 4T reference hole in cn ASME No.12 penetrameter and the acceptance criteria specified.
Steam Generator No. 1 - 94-1A, 94-2A, 94-3A, 94-4A, 94-8A,94-12A Steam Generator No. 2 - 94-1B, 94-2B, 94-5B, 94-6B, 94-78, 94-8B Steam Generator No. 3 - 94-lC, 94-3C, 94-4C, 94-5C,94-10C, 94-14C Steam Generator No. 4 - 94-10, 94-2D, 94-4D, 94-7D, 94-94-9D,94-110 No items of noncompliance were identified.
5.
Procedure Review The following procedures were reviewed for compliance to the requirements of ASME 1974 Summer 1975.
-
YA - WP5 Rev. 3 - Gas tungsten Arc Welding (GTAW) and Shielded Metal Arc welding (SMAW) of carbon steel (Pl to P1) with open root, also the procedure qualification was reviewed.
-
YA - WP8 - General requirements for GTAW and SMAW welding of carbon steel and austenetic steel P8.
-
H.S.B. Hartford Steam Boiler inspection and insurance procedure for radiography inspection.
No items of noncompliance were identified.
6.
Documentation Randomly selected records of six welds in each of the feedwater lines for the four steam generators (24 welds total) where the welding, testing and inspection was completed were examined to determine whether the records conform to established procedures and the work accomplished was consistent with NRC requirements and the ASME code commitments.
The type of documents examined for each of the 24 welds are listed below:
-
Welder Identification and Qualification Material Identification
-
-
.
.
Weld procedure used
-
-
-
Inspection of fit - up
-
X-ray of root layer of weld
-
X-ray of final weld Liquid Penetrant final weld
-
-
Ultrasonic Examination for Pre-Service Baseline data No items of noncompliance identified.
7.
Weldor Qualification The inspector audited the qual'fication records of four welders for compliance to ASME Section IX.
The welder symbols are listed below:
