Routine unannounced inspection by a regional based inspector concerning licensee's actions required by IE Bulletins 79-02,79-06C and 79-13.

Followup on IE Bulletin 79-02 The inspector performed a review of licensee's actions regarding the testing of concrete expansion anchors and support base plates identfied in this Bulletin to verify that inspections and tests were being con-ducted as required by IE Bulletin 79-02 (and supplements) and in ac-cordance with licensee's responses dated July 20 and August 13, 1979.

Review of Test Procedures The inspector reviewed the following:

This review was performed to verify that inspection elements required by IEB 79-02 were included and that appropriate acceptance criteria were applied.

Since there items may bear on flexibility studies at some later date, the licensee agreed to record this data.

No other inadequacies were identified.

Observation of Testing The inspector accomoanied a team on a test of four anchors in the pressurizer cubici: to verify that test personnel were performing the tests in accordance with the approved procedures.

No inadequacies were identified.

Status of Test Program In the licensee's response dated August 13, 1979, a sample size of 20% of the hangers in each safety related system was established as a starting point which could be expanded dependent on test results.

Testing to date identified a significant failure rate of the Red-Head type anchors (7 of 16 pressurizer safety valve piping, and 2 of 4 on shutdown cooling piping).

Based on this failure rate the licensee has expanded the test program to include 100% of the Red-Head anchors.

No failures of other anchors have been identified.

The inspector reviewed the maintenance documentation to verify that the failed anchors were correctly replaced.

The inspector discussed with the licensee that the justification of 20% sample for the remaining anchors had not been established to demonstrate the 95% confidence level addressed in the Bulletin.

The licensee is continuing a review in this area and this will be reviewed in a subsequent inspection.

IE Bulletin 79-06C The inspector verified that the licensee's emergency procedures listed below had been revised to require immediate tripping of main coolant pumps following a loss of coolant accident which results in initiation of safety injection.

No inadequacies were identified.

IE Bulletin 79-13 During the performance of volumetric testing of welds in high stress areas of feedwater piping, the licensee identified code defects in the steam generator nozzle welds on all four steam generators which require re-welding to repair.

assemblies are also suspect, the licensee is proceeding with plans to completely replace the piping associated with these loop seals prior to startup.

At the time of this inspection. it had not been determined that the highest stress areas in the feedwater lines were in this

section of piping.

The licensee has agreed to perform an evaluation to determine if testing of other areas should be performed oecause of higher stresses.

This will be reviewed during a subsequent inspection.

5.

Plant Tour The inspector performed a tour of accessible areas of the plant including the Vapor Container, Turbine Building, Primary Auxiliary Building, Radwaste Building, Diesel Generator Rooms, Switchgear Room, and Control Room.

6.

Exit Interview The inspector met with licensee representatives (see detail 1 for attendees) at the completion of the inspection to discuss the scope and findings of the inspection as detailed in this report.
