Inspection on November 5, 1980 (Report No. 15000027/80-66)

As part of the inspection conducted on November 5, 1980, the inspection of part of trailer 44578 is detailed in this report.

Because the shipment arrived the week prior to the inspector being on site and the final disposition of the shipment was made the week after the inspector left the site, the inspector

can only detail what he saw personally, mainly the two drums with the loose lids. Otner information in this report was obtained from the State of Nevada Inspector's report.

A representative from the Department of Transportation was not present during this inspection.

Burt Grey from the Nevada State Health Division was present during this inspection.

Results:

One apparent item of noncompliance with D0T/NRC regulations was observed. See paragraph 2.

It appears that the NRC Region I Office, the State of Nevada and the Department of Transp tation are the appropriate organizations to take enforcement actions.

Because the truck arrived the week prior to the inspector being on site and the final disposition of the shipment was made the week after the inspector left the site, no surveys were made.

Surveys were made by the State of Nevada inspector.

Some information in this report was obtained from the State Inspector's report.

This shipment consisted of 66 drums of LSA radioactive material, no other packages were on this truck other than those shipped to the disposal site.

2.

(a) During the partial inspection of trailer number 444578 it was noted that drum number 107 had the lid raised approximately

3/4-inch and tne lock ring was not secure.

The bolt which was

to secure the lock ring and lid appeared to have been backed off, leaving the lid and lock ring loose.

-2-(b) The lock ring of drum number 69 was loose.

The above situations appear to be in noncompliance with 49 CFR 173.392 (c)(1) which requires that materials must be packaged in strong, tight packages so that there will be no leakage of radioactive material under conditions normally incident to transportation.

All radiation surveys of the vehicle, and checks for removable contamination were conducted by the State of Nevada inspector. All results were acceptable.
