In accordance with ASME Section XI, paragraph IWA 6240, enclosed is the Diablo Canyon Power Plant Unit 1 Fifteenth Refueling Outage Inservice Inspection (ISI) Report. This report provides the results of the ISI examination of Class 1, 2, and 3 components and supports, Reactor Top Head Volumetric and Visual, and Reactor Bottom Visual inspections.

There are no new or revised regulatory commitments as defined by the Nuclear Energy Institute 99-04, "Guidelines for Managing NRC Commitment changes,"

dated July 1999, in this report.

Enclosure PG&E Letter DCL-09-046 SECTION A INTRODUCTION Inservice inspection (ISI) of scheduled ASME Section XI components and their supports at Pacific Gas and Electric Company's Diablo Canyon Power Plant (DCPP) Unit 1 was completed during the fifteenth refueling outage from January through March, 2009. The DCPP Unit 1 fifteenth refueling outage (1 R15) was the second outage in the first period within the third 10-year ISI interval.

This report is submitted as required by ASME Section XI, paragraph IWA-6240, and is organized in several sections. An introduction is provided in Section A. Section B contains the completed ASME Code Data Report Form NIS-1 for Class 1, 2, and 3 components. Sections C and D detail the Class 1, 2, and 3 examinations performed, date of examination (for group items or for more than one type of examination performed, the date provided is the most recent examination date), any unusual conditions noted, corrective actions if required, and explanatory remarks.

System pressure tests on Class 1, 2, and 3 systems conducted during the period following 1R14, and up to and including 1R15, are reported in Section E.

Appendix J, containment penetration pressure tests are reported in Section F.

Preservice inspections were performed on certain components, as required after repairs or replacements. These items are detailed in Section G.

Section H of this report, Repair / Replacement Activities, contains the completed ASME Code Data Report Form NIS-2 for 1 R15 work completed prior to and including May 29, 2009 and for any mid cycle and 1R14 packages closed after June 30, 2007.

NIS-2 reports for 1R15 Repair / Replacement work identified after May 29, 2009 due to a lag in the addition of work completion remarks, will be included in the next outage report.

Corrective actions and order package documentation are on file at DCPP and are referenced by order number in the NIS-2 reports.

Manufacturers' Data Reports for 'New Items' are attached behind the applicable NIS-2.

In addition to the ASME Code examinations reported here, the following Reactor Vessel examinations were performed during 1R15:

Reactor Vessel Top Head Volumetric and Visual Examinations:

On September 10, 2008, the Federal Register published the Final Rule 10 CFR 50.55a(g)(6)(ii)(D)(3) establishing ASME Section XI Code Case N-729-1, with additional conditions stated in 10 CFR, as the inspection requirement for reactor pressure vessel heads at pressurized water reactor facilities.

Enclosure PG&E Letter DCL-09-046 DCPP Unit 1 has head penetration nozzles that are fabricated from UNS N06600 material with UNS W86182 welds. Therefore, Pacific Gas and Electric Company (PG&E) was required to perform inspections in accordance with ASME Section XI Code Case N-729-1, Category Items B4.10 and B4.20.

During IR15, PG&E complied with ASME Section XI Code Case N-729-1 Category item B4.10 by performing a bare metal visual examination of the reactor pressure vessel (RPV) head penetrations, including 360 degrees around each of the vessel head penetration (VHP) nozzles and the head vent penetration. The bare metal visual inspection to identify degradation covered greater than 95 percent of the RPV head surface as determined using the formula in the Code Case. The only area not covered by the bare metal visual inspection was that area inaccessible due to support structure interference. However, the inspection included those areas of the RPV head upslope and downslope from the support structure interference. No evidence of boron or corrosion product was found and no evidence of VHP nozzle leakage or cracking, or degradation of the RPV head was identified.

During I R15, PG&E also complied with Category item B4.20 by performing a nonvisual nondestructive volumetric examination (NDE) on all 79 reactor head penetration tubes (RHPT) and the head vent penetration. A combination of ultrasonic (UT) and eddy current testing methods was used with probes delivered to the tubes by a remote positioning device (or manually in the case of the head vent tube).

The inspection coverage above and below the J-groove weld satisfied the Code Case requirements for all penetrations.

On December 4, 2008, PG&E requested relief from the Code Case conditioning requirement of 10 CFR 50.55a(g)(6)(ii)(D)(3) requiring use of a "demonstrated volumetric or surface leak path assessment through all J-groove welds" (ref PG&E letter DCL-08-103).

The NRC approved relief from this requirement for DCPP Unit 1 by letter, "Diablo Canyon Power Plant, Unit No. 1 - "Approval of Request for Relief from 10 CFR 50.55a(g)(6)(ii)(D)(3) Requirement for Demonstrated Volumetric Leak Path Assessment" dated April 8, 2009. The examination conducted was within the provisions of the approved relief request.

The examinations on all tubes detected no discontinuities or indications of boric acid leak paths, no flaws needing engineering disposition, and no corrective actions were identified.

One other reactor top head OE issue experienced at other facilities was addressed during the. 1 R15 examination, regarding wear on thermal sleeves at the interface with the control rod drive mechanism tube. All thermal sleeves were examined for wear using high 5

Enclosure PG&E Letter DCL-09-046 resolution cameras mounted on the inspection robot. No significant wear was detected.

No further evaluations were required.

Reactor Vessel Bottom Head Visual Examinations:

On September 10, 2008, the Federal Register published the Final Rule 10 CFR 50.55a(g)(6)(ii)(E)(1) establishing ASME Section XI Code Case N-722-1, with additional conditions stated in 1 OCFR, as the inspection requirement for reactor pressure vessel bottom heads at pressurized water reactor facilities.

In accordance with the requirements of ASME Section XI'Code Case N-722-l, Category Item B 15.80, the 58 reactor vessel bottom head penetrations were visually examined for evidence of pressure boundary leakage. This was accomplished by use of a robotically deployed inspection camera system whose resolution was confirmed to be equivalent to bare metal direct visual (VE) examination. The Unit 1 bottom head insulation package.

had been previously modified to permit access with this equipment. All 58 bottom mounted instrumentation tube penetrations were examined with no limitations encountered. No evidence of pressure boundary leakage was detected.
