Crystal River Unit 3 - Special Report: Seismic Monitoring Instrumentation Inoperability

Dear Sir:

Florida Power Corporation (FPC), doing business as Progress Energy Florida, Inc., hereby submits this Special Report pursuant to the requirements of the Crystal River Unit 3 (CR-3) Final Safety Analysis Report (FSAR), Section 2.5.4.4, Seismic Monitoring Instrumentation. The CR-3 FSAR requires that if one or more of the listed seismic monitoring instruments become inoperable for more than thirty (30) days, then a Special Report is submitted to the NRC within the next ten (10) days. The Special Report is required to include the cause of the malfunction and the plans for restoring the instrument(s) to a functional status.

The instrumentation subject to this report is the Triaxial Time-History Accelograph channel for Outside Containment on Top of Ring Girder (SI-2-MAT). This was discovered to be failed during the monthly functional test. On June 24, 2010 when the failure was discovered, Nuclear Condition Report (NCR) 406657 was generated in the CR-3 Corrective Action Program to document this condition.

The apparent cause of the malfunction of this instrumentation was an equipment failure when the cable to the accelerometer was pulled out of the junction box and three wires were broken. This damage occurred when the reactor building temporary construction trolley crane came in contact with the cable. The instrumentation was returned to service about five (5) hours past the thirty (30) days on July 24, 2010.
