Dear Sir:

In the referenced letter, Florida Power Corporation, doing business as Progress Energy Florida, Inc. (PEF), submitted Special Report 03-01 containing the Once Through Steam Generator (OTSG) inspection notifications required prior to MODE 4. During a telephone conference with PEF on July 8, 2004, the NRC discussed a Request for Additional Information (RAI) regarding the subject special report. RAI questions 1 and 2 of the RAI and the corresponding PEF responses are provided in Attachments A and B to this letter. The response to RAI questions 3 and 4 will be submitted to the NRC in September 2004.

This letter establishes no new regulatory commitments.

U.S. Nuclear Regulatory Commission Attachment A 3F0804-04 Page 1 of 51 RAI Ouestion 1 The B&W Owners Group Technical Report, "Alternate Repair Criteria for Tube End Cracking in the Tube-To-Tubesheet Roll Joint of Once Through Steam Generators" (BAW-2346P) provides the bases for the steam generator (SG) tube end cracking (TEC) alternate repair criteria (ARC) used by the licensee. This report provides the methodology for assessing leakage from axial TEC indications with leakage values being assigned to indications as a function of the tubesheet radius. For each indication detected provide the row/column number, tubesheet radius, the location of the indication and the leakage value.

In addition, provide a summary table listing the number of indications detected at each discrete tubesheet radius listed in table 7-6 of BAW-2346P. Include in this response a tubesheet map.

Response

Tables A-1, A-2, B-i, and B-2 provide a list of each Tube End Crack (TEC) detected during the Fall 2003 Once Through Steam Generator (OTSG) inspection. As identified in this letter, TECs are only those crack-like tube end indications which meet the requirements to remain in service based on the approved ARC. The four tables list each TEC tube by row & column number, tubesheet radius, location (upper or lower tube end), number of indications per tube, and the total assigned leakage value. The tables are separated by OTSG and upper/lower location. Summary Tables A-3 and B-3 provide a listing of the number of indications detected at each discrete tubesheet radius listed in Table A-6 contained in the May 28, 1999 letter from Florida Power Corporation to the NRC. Figures 1, 2, 3 and 4 contain tubesheet maps showing the location of as-found TECs from each OTSG.

U.S. Nuclear Regulatory Commission Attachment B 3F0804-04 Page 1 of 2 RAI Ouestion 2 The estimated leakage associated with the as-found conditions during the 2003 outage exceeded projections made during the 2001 outage. This is the case for both the lower tubesheet (LTS) and the upper tubesheet (UTS) indications. The under prediction can be attributed In part to the detection of indications in the LTS; however, the leakage associated with indications in the UTS was also under predicted. These results imply that the leakage methodology may not be adequate e.g., the probability of detection (POD) at Crystal River 3 may be less than the generic value provided in BAW-2346P. As a result of the above, discuss the cause of the under prediction of the leakage associated with the UTS indications and your corrective actions. In addition, discuss how this affects your projections for the amount of leakage expected during your next steam generator inspection.

Response

The POD reflects the ability of the inspection method to detect all of the tube end cracks (TEC) that exist in the steam generator tubing. For the TEC Alternate Repair Criteria (ARC), the number of TEC indications found during eddy-current inspections is scaled upward by a factor of 1/POD to account for non-detected cracks which could potentially leak during accident conditions in the next cycle of operation. The POD used at CR3 is 0.84 as described in BAW-2346P. POD is applied by taking the number of indications found during an inspection and increasing by the inverse of the POD. The amount of additional POD leakage, in addition to the as-left TEC leakage, is then projected to exist during the following operating cycle.

CR3 has performed a 100% upper tube end inspection during the previous three refueling outages (1JR, 12R, & 13R) and existing TEC indications are well known. Use of the POD to account for future TEC indications should be adequate if there are only a small number of new indications that develop during the operating cycle. However, the POD was not originally intended to predict the initiation of new TECs that were not previously at the tube ends. For example, during the 13R outage new TEC indications (upper and lower combined) in the "A" Once Through Steam Generator (OTSG) made up 24.6% of the total number of indications and 31.4% in the "B" OTSG.

Section 9.0 of BAW-2346P only describes the assessment of growth as it relates to an axial indication growing in length and the distance to the Clad-to-Carbon Interface (CCI). The measured growth of existing axial TECs towards the CCI indicates an insignificant amount of increase during the previous three inspections. PEF addressed the issue of new indications by providing additional margin in the leakage value. This was accomplished by re-rolling a number of tube ends which made the as-left leakage plus growth numbers acceptable.

During the 13R outage, when it was identified that the TEC leakage had exceeded the leak rate limit, an internal Non-Conformance Report (NCR) was generated. The root cause evaluation identified the following as the primary cause:

"The use of the TEC POD does not provide sufficient margin to account for the increase in the number of new TECs at the upper tube end found in future outages.

This lack of awareness is the main reason for the failure to predict the increased TEC leakage and thereby exceeding the as-found MSLB leakage limit of 0.856 gpm.

The corrective actions for this issue include:

1. The OTSG portion of the Reactor Coolant (RC) System was classified as being (a)(1) in the Maintenance Rule in accordance with the CR3 Maintenance Rule Program and NEI 97-06 Rev. 1 (Steam Generator Program Guidelines).

2. During 13R, CR3 plugged or re-rolled as-found tubes with high postulated TEC leak rates to reduce the 13R as-left TEC leakage and provide a margin for future new TECs. A total of 162 tubes were repaired in the "A" OTSG and 182 tubes were repaired in the "B" OTSG. The tubes were chosen from the high leak rate zones and tubes with multiple indications. The goal was to reduce the as-left TEC leakage contribution to provide an acceptable minimum margin (> the original POD value) for operation until Refueling Outage 14 (14R).
3. Provide this Operating Experience (OE) to other utilities for their information. This action is complete. Due to the limited applicability in the industry, the OE was discussed with the other OTSG plants at the Babcock & Wilcox Owners Group (BWOG) Steam Generator Committee meeting during December 3-5, 2003.
4. Revise the CR3 Steam Generator Program Manual to include an expanded section to provide information on the CR3 specific TEC leakage tables to ensure the TEC calculations are performed correctly. This section will also identify that the Operational Assessment must consider the past (historical) growth, and applicable leakage rates when predicting future cycle operation and not just rely on predictions based on POD.

The manual was revised to identify that prior to a planned OTSG inspection, the expectations for using the correct TEC leak tables, including new TEC initiation rates used for the Operational Assessment, will be provided to the vendor performing the Condition Monitoring (CM) and Operational Assessment (OA) evaluations. The changes included:

  • CR3 specific TEC Leak Rate vs. Radius Table for the "Faulted" steam generator will be from Table A-6 (Florida Power Corporation letter to the NRC, dated May 28, 1999). The OA needs to include TEC historical growth rates in numbers of indications.
  • Additional margin for TEC leakage must be provided beyond the amount calculated for POD.
5. The BAW-2346P report contains several very conservative assumptions about the TEC leak rates along with the assumption that every indication is 100% thru-wall and leaks.

For example, the topical report predicts the primary-to-secondary leakage per TEC indication during normal plant conditions. Compared to the actual OTSG primary-to-secondary leakage, the predicted leakage for the previous operating cycle would have been over 40 gallons per day compared to an actual of 2.5 gallons per day. In addition, even though new TECs appear to develop over the operating cycle, the actual primary-to-secondary leak rate has not increased during the two previous operating cycles.

Therefore, one of corrective actions that CR3 intends to pursue is to have Framatome ANP review the leak rate assumptions identified in BAW-2346P and evaluate an alternative method such as the Generic Letter 95-05 approach to be able to utilize more realistic TEC leakage rates. This action is ongoing.

As a result of the internal investigation, it was recognized that the additional amount of assumed leakage from the POD calculation in BAW-2346P was not sufficient to account for the leakage from new indications. Therefore, the projections for TEC assumed leakage for the next steam generator inspection had to be increased by an additional amount to compensate for the potential number of new indications.
