Entergy Operations, Inc. (Entergy) inspected the Arkansas Nuclear One, Unit 1 (ANO-1) steam generator (SG) tubes during the Fall (October) 2011 refueling outage (1R23) in accordance with ANO-1 Technical Specification (TS) 5.5.9, Steam Generator (SG) Program.

ANO-1 TS 5.6.7 requires that the results of inspections performed during the report period be submitted to the NRC within 180 days after the initial entry into Mode 4. Enclosed is the subject inspection report.

The 1R23 inspection performed on both SGs involved an initial full-length bobbin coil examination. The X-probe used consists of an array of coils for diagnostic testing.

This submittal completes the reporting requirements of the ANO-1 TSs for this inspection.

Enclosure to 1CAN031203 Page 1 of 13 RESULTS OF ANO-1 SG TUBE INSPECTIONS DURING 1R23 1 INTRODUCTION Arkansas Nuclear One, Unit 1 (ANO-1) Technical Specification (TS) 5.6.7, Steam Generator Tube Inspection Reports, requires Entergy Operations, Inc. (Entergy) to submit a 180 day report to the NRC that outlines the details of the steam generator (SG) tubing inspections that were performed during the reporting period. The report shall include:

1. The scope of inspections performed on each SG.
2. Active degradation mechanisms found.
3. Nondestructive examination (NDE) techniques utilized for each degradation mechanism.
4. Location, orientation (if linear), and measured sizes (if available) of service induced indications.
5. Number of tubes plugged during the inspection outage for each active degradation mechanism.
6. Total number and percentage of tubes plugged to date.
7. The results of condition monitoring, including the results of tube pulls and in-situ testing.

The operating period for this report includes one refueling inspection outage (1R23) in October 2011. The previous inspection included only the tubes around the tie rods (1R22).

The purpose of the 1R22 inspection was to evaluate the extent of tie rod bowing. The detected wear at the tube support plates (TSPs) permitted omitting this inspection in 1R23.

The outage prior to 1R22 (1R21) included a 100% bobbin inspection.

This report details the result of the 1R23 inspection. The inspection was a 100% full-length bobbin inspection, as well as utilizing the X-probe for diagnostic testing. Three damage mechanisms were identified (two previous and one new). Tie rod bowing and TSP wear has been identified in both SGs previously. Tube-to-tube wear has now been identified in both SGs for the first time. This condition was entered into the site corrective action program. No identified damage mechanism challenged the performance criteria for leakage or burst.

2 DESIGN The replacement SGs for ANO-1 are Enhanced Once-Through Steam Generators (EOTSG) manufactured by AREVA. The EOTSG is a straight shell-and-tube type heat exchanger installed in a vertical position with bottom supports and emergency supports as required to accommodate normal and accident loads. The tubing consists of Inconel 690 thermally treated tubing that is 5/8" in diameter with a 0.037" wall thickness. The tubes are expanded full depth hydraulically in the tubesheet. There are 15 TSPs that are constructed of stainless steel (SA 240 type 410) and are a broached trefoil-hole design.

1R23 was the fourth inservice inspection (ISI) following replacement of the SGs. There were three mechanisms identified (mechanical wear at TSPs, tube-to-tube wear and tie rod/tube bowing). These are listed in Table 3.2.1.

The only service induced mechanisms were mechanical wear at the TSPs and tube-to-tube wear. There were no wear indications associated with proximity tubes (tubes in close contact or contacting adjacent tubes due to tie rod bowing). Due to the large number of indications, these are listed in the following attachments:

There were 16 tubes plugged in 1R23 with 15 of these tubes being stabilized. The tube that could not be stabilized was caged with the surrounding tubes by plugging and stabilizing those tubes. Listed below in Tables 3.5.1 and 3.5.2 is a summary of the plugged tubes for SG A and SG B, respectively:

There were no sleeves installed in either generator. The aggregate plugging information is listed in Table 3.6.1 below.

All condition monitoring requirements for the tubing performance criteria were met. There were no indications that exceeded performance criteria or in-situ screening limits. No tube pulls were required.

Proximity is an indicator of tube bowing. The bowing resulted from tie rod bowing due to a compressive load applied during cool down from locking of the TSPs in the upper shroud.

This was originally presented to the NRC in a presentation on August 29, 2007 (a non-proprietary version of the presentation material provided for the meeting can be found in ADAMS (ML072420379)). The results of the NDE inspection of SG A, which included a full-length bobbin inspection of 100% of the tube bundle and inspection of the tubes surrounding each of the tie rods with an X-Probe (combination array probe which includes bobbin coils and diagnostic coils), indicated a decrease in the number of affected tubes compared to 1R22. Five new tube-to-tie rod proximity indications were detected, one in the 1st span, two in the 14th span, and two in the 15th span. No additional tube-to-tube proximity indications were detected. The general pattern was the same as that of the previous inspections with the addition of one location in the third ring (from the periphery) of tie rods in the first span. One location in the 15th span indicated progression to tube-to-tie rod contact.

Enclosure to 1CAN031203 Page 6 of 13 The results of the NDE inspection of SG B, which included a full-length bobbin inspection of 100% of the tube bundle, included proximity indications in SG B for the first time. This was entered into the site corrective action program. Each of the tubes that had proximity signals on the bobbin data were subsequently inspected with an X-Probe (array/bobbin combination).

A review of the eddy current testing (ECT) data from 1R22 did not reveal any signals that were consistent with tube-to-tie rod or tube-to-tube proximity. Therefore, the tie rod bowing at these locations was reasonably concluded to be new. There were seven locations of tie-rod-to-tube contact in the 1st span of SG B, compared to eleven locations of tie-rod-to-tube contact in the 1st span of SG A at 1R20.

Listed below in Tables 3.7.1 and 3.7.2 are the lateral bowing extents by location for the last four outages.

Enclosure to 1CAN031203 Page 11 of 13 TSP Wear During 1R23, a significant number of new wear indications were detected and sized using a wear standard. A review of the previous ECT data was performed to confirm that newly reported wear indications were actually new. The inspection during 1R22 was limited to tubes surrounding the tie rods; therefore, the 1R23 wear data was compared to the 1R21 wear data to evaluate growth and trending. The wear patterns in each of the SGs were essentially unchanged, though there appeared to be a new pattern of appreciable wear at TSP 6 in SG A. The total number of wear indications in SG A has increased by approximately 51%.

The total number of wear indications in SG B has increased by approximately 37%. The maximum depth during 1R23 was 46% TW in SG B compared to 32% TW in both SGs during 1R21.

There has been no significant change in the distribution of wear as a function of tubesheet radius since 1R21, with the majority of the wear indications at a radius greater than 35 inches from the center of the bundle. There were a few new wear indications in the lower spans; however, the depth of these indications was less than 10% TW. Otherwise, the distribution of wear indications as a function of the TSP elevation was similar to that indentified in previous inspections. New wear indications were characterized with 93% of the new indications in SG A and 96% of the new indications in SG B having a flat orientation. The wear depths during 1R23 were slightly deeper than past inspections because all of the indications have had two cycles of operation to grow, with only a small percentage taken out-of-service by plugging. Table 3.7.3 summarizes the TSP wear:

The detailed listing of individual indications is found in Attachment 3 for SG A and for SG B.

Tube-to-Tube Wear A meeting with the NRC was conducted on January 26, 2012 (a copy of the ANO-1 slides can be found in ADAMS (ML120250111)), to discuss this new mechanism in conjunction with TMI.

Both ANO-1 and TMI performed SG inspections during the fall of 2011. Numerous freespan (not coincident with a tube support or tubesheet) bobbin coil Absolute Drift Indications were detected at the first ISI of the SGs at TMI. Scrutiny of these determinations resulted in the conclusion that these were actually indications of tube-to-tube wear. Further diagnostic tests with +Point' and X-probes were performed to confirm the presence of wear indications in the mid-span regions of the SG. Most of the wear indications were detected in either the 8th or 9th spans of the tube bundle, mid-span between broached TSPs. There were no corresponding indications of tube-to-tube proximity caused by bowing of the affected tubes. Based on the findings at TMI, a complete reanalysis of the ANO-1 bobbin coil data was performed by AREVA beginning with the first ISI during 1R20. The evaluation determined that during 1R23, there were 48 tubes in SG A and 78 tubes in SG B, for a total of 126 tubes with indications. A

%TW depth was assigned using a bobbin voltage correlation based on the TMI data, where sizing was obtained using qualified techniques. There were 25 indications in SG A and 11 indications in SG B that had no signal present based on a review of the first ISI data (1R20).

Eight new indications in SG A and three new indications in SG B were detected during 1R21 and repeated during 1R23. The reanalysis of the bobbin data shows that the mechanism of tube-to-tube wear is active at ANO-1, as well as TMI. It also illustrates that indications do not occur only in the first cycle and then remain unchanged over subsequent operating periods.

However, the 1R23 depth sizes demonstrate that the indications were not significantly active.

The two indications with the largest depths (26% TW in SG B) were essentially unchanged

Enclosure to 1CAN031203 Page 13 of 13 from 1R21. There were two locations in SG A that showed two wear scars approximately 60 degrees apart on the array data. There were six locations in SG B with two wear scars and two locations with three wear scars, all approximately 60 degrees apart on the array data.

The reported depth was the total depth for the two wear scars based on the bobbin correlation. Attachment 1 is the listing of indications in SG A and Attachment 2 is the listing of indications in SG B.
