| Description | Technical Specification Section 5.4.1.a requires, in part, that written procedures shall be established, implemented, and maintained covering the applicable procedures recommended in Appendix A to Regulatory Guide 1.33, February 1978. Regulatory Guide 1.33, Appendix A, Section 7(e) requires, in part, that radiation protection procedures for access control to radiation areas be established, including a radiation work permit system. Radiological Work Permit 2011-0404 implemented this requirement and Task 1 of this permit allowed access to high radiation areas, but required that workers receive authorization to access beneath the grating. Contrary to the above, on September 28, 2011 a radiation worker failed to implement Radiological Work Permit 2011-0404 by failing to receive authorization from radiation protection prior to entry below the grating. The violation is considered to be of very low safety significance because: (1) it is not an ALARA planning issue, (2) there was no overexposure, (3) there was no substantial potential for overexposure, and (4) the licensees ability to assess dose was not compromised. The licensee placed the finding into the corrective action program as Condition Report CR-2011-010601. |
