Title | Failure to Perform Preventative Maintenance on the Supplemental Emergency Power System |
Description | The team identified a finding of very low safety significance, in that NextEra did not perform preventative maintenance (PM) on supplemental emergency power system (SEPS) electrical components as required by the approved engineering design modification for SEPS. As a result, the system\'s reliability to respond to a loss of off-site power event had not been maintained at a high confidence level, as assumed in NextEra\'s design and probabilistic risk analyses. In response, NextEra entered the issue into their corrective action program, evaluated the effect on equipment reliability for the never-performed PMs, and implemented an accelerated schedule to complete the missed PM tasks. The finding was more than minor because, if left uncorrected, it had the potential to lead to a more significant safety concern. In addition, the finding was associated with the Procedure Quality and Equipment Performance attributes of the Mitigating Systems Cornerstone and affected the cornerstone objective to ensure the availability, reliability, and capability of systems that respond to initiating events. The team determined the finding was of very low safety significance because it was not a design or qualification deficiency and did not result in the loss of the SEPS system or train function. This finding had a cross-cutting aspect in the area of Human Performance, Decision Making, because the most recent quarterly system health report (4th quarter 2012) had stated that SEPS PMs had not been scheduled or performed, these reports had been reviewed by NextEra management, however actions were not taken to develop electrical PMs on the SEPS. |
