Failure to Implement Adequate Steam Generator Blowdown Demineralizer Control Procedures

Description

A self-revealing Green NCV of Technical Specifications (TS) 6.8.1.a, Procedures and Programs, was identified for licensees failure to establish and implement adequate steam generator blowdown demineralizer control operating procedures resulting in exceeding secondary water chemistry Action Level 3 criteria for impurities in the steam generators. Specifically, the licensee did not implement adequate isolation valve controls between the demineralizer resin regeneration system and the feedwater system during resin regeneration activities. This open path allowed leakage of sulfates and chlorides into the feedwater system. The level of these impurities exceeded the secondary chemistry Action Level 3 threshold and resulted in an unplanned shutdown.
