| Title | A ESW Pump Power Supply Cables Submerged in Water |
| Description | The inspectors identified a Green NCV of 10 CFR Part 50, Appendix B, Criterion III, Design Control, in that the licensee failed to maintain the A ESW pump power cables in an environment for which they were designed. Specifically, the cables were submerged in water in manway 73B-SA, a condition for which they were not qualified. The licensee entered this issue into the CAP as AR #376709. As immediate corrective action, the licensee pumped the manway dry. The inspectors determined that the failure to ensure that the A ESW pump power cables were maintained in an environment for which they were designed was a performance deficiency. The finding was more than minor because, if left uncorrected, it had the potential to lead to a more significant safety concern. Specifically, it could have caused the A ESW pump to become inoperable in the event that the cable failed due to long term degradation as a result of continuous submergence. The finding affected the equipment performance attribute of the Mitigating Systems cornerstone objective to ensure the availability, reliability, and capability of systems that respond to initiating events to prevent undesirable consequences. The inspectors evaluated the significance of this finding using IMC0609, Significance Determination Process, Phase 1 Worksheet. The finding was of very low safety significance because it was a qualification deficiency that did not result in a loss of operability. This finding was determined to have a cross-cutting aspect in the CAP component of the Problem Identification and Resolution area associated with timely and effective corrective actions |
