| Description | Technical Specification 5.4.1.a, Procedures, requires, in part, that written procedures be established, implemented, and maintained as recommended in Regulatory Guide 1.33, Revision 2, Appendix A, dated February 1978. Paragraph 9.a of Regulatory Guide 1.33, Appendix A, requires that maintenance that can affect the performance of safety-related equipment should be properly preplanned and performed in accordance with written procedures, documented instructions, or drawings appropriate to the circumstances. Contrary to the above, on July 3, 2012, Work Order 01196946-02 was not appropriate to the circumstances because it did not include seismic design considerations and did not provide reference elevations for workers performing excavation near service water valve SW-V-933A. Consequently, the licensee on July 3, 2012, over-excavated near SW-V-933A when performing buried piping inspections. This finding was identified by the licensee and entered in the licensees corrective action program as Action Requests AR 266400266400and 266405. This finding was determined to be of very low safety significance because it was a design or qualification deficiency confirmed not to result in a loss of operability. |
