10 CFR Part 50, Appendix B, Criterion XVI, Corrective Action, requires, in part, that measures be established to assure that
conditions adverse to quality, such as deviations and nonconformances are promptly identified and corrected. Contrary to this requirement, from April 2007 through January 2009, the licensee failed to establish measures to assure that a
condition adverse to quality was promptly identified and corrected. Specifically, the licensee failed to identify that an incorrectly designed transformer subject to overheating and failure had been installed in the Class 1E power system and, following identification in January 2009, the licensee failed to promptly correct the
condition adverse to quality. This performance deficiency was of very low safety significance (Green) because it did not represent a loss of system safety function, did not represent the actual loss of safety function of a single train for greater than its technical specification allowed outage time, and did not screen as potentially risk significant due to a seismic, flooding, or severe weather initiating event. This issue was entered into the licensees corrective action program as Action Request/Condition Report 0204769
