| Title | Failure to Replace Defective Fuses in the A EDG Resulting in Generator Failure |
| Description | The inspectors identified a Green NCV of 10 CFR 50, Appendix B, Criterion XVI, associated with Dominions failure to prevent recurrence of a significant condition adverse to quality, installation of defective fuses in the Unit 2 emergency diesel generators (EDGs) from September 26, 2015, until October 23, 2015. Dominions immediate corrective actions included replacing the defective fuses in both EDGs and assessing the extent of condition in other safety systems. This finding was determined to be more than minor in accordance with IMC 0612, Power Reactor Inspection Reports, Appendix B, Issue Screening, as it represented a challenge to the equipment performance attribute of the Mitigating Systems cornerstone objective to ensure the availability, reliability, and capability of systems that respond to initiating events to prevent undesirable consequences. This finding screened to be of very low safety significance (Green) because the finding did not represent an actual loss of function of a single train for greater than its allowable outage time. The inspectors assigned a cross-cutting aspect in the Problem Identification and Resolution, Operating Experience, in that Dominion failed to effectively implement relevant internal and external operating experience. |
